摘 要:目的分析贵阳地区262例发热患者急性上呼吸道病毒感染谱,探讨呼吸道病毒多联检测在疫情防控中的价值,为临床诊疗及防治提供依据。方法使用多重荧光定量PCR方法对2020年7月至2021年1月贵阳市第一人民医院发热门诊就诊的发热患者采集的咽拭子样本进行16重常见的呼吸道病毒谱检测。结果 262例发热患者SARS-CoV-2核酸均为阴性,有158例感染了其他呼吸道病毒(占60.7%),其中以肠道病毒24.4%(64/262)、呼吸道合胞病毒14.9%(39/262)、副流感类病毒10.7%(28/262)感染为主。对上呼吸道感染患者不同年龄组分析,发现肠道病毒、呼吸道合胞病毒对儿童更易感染;混合感染更易发生于≤6岁儿童组;副流感3病毒感染患者的体温多低于39℃,而人偏肺病毒感染者的体温更易超过39℃。结论肠道病毒、呼吸道合胞病毒及副流感类病毒是贵阳地区发热患者上呼吸道感染的主要病毒病原体,在疫情期间发热患者早期鉴别诊断中通过对呼吸道病毒的多重PCR检测,可提供更为精准的防控策略。Objective To analyse the virus spectrum of 262 fever patients with acute upper respiratory tract infection in Guiyang, to explore the value of multiple respiratory virus detection in the epidemic prevention and control and to provide evidence for clinical diagnosis and treatment. Methods 16 multiple respiratory pathogens were used to detect the fever outpatient pharyngeal swab samples collected from the fever clinic of the first people’s hospital of Guiyang from July 2020 to January 2021. Results The SARS-CoV-2 nucleic acid of 262 fever patients was negative. There were 158 cases of co-infection with other respiratory viruses(60.7%), among which 24.4%(64/262) were enterovirus, 14.9%(39/262) were respiratory syncytial virus and 10.7%(28/262) were parainfluenza virus. By comparing different age groups of patients with upper respiratory tract infection, we found that enterovirus and respiratory syncytial virus were more susceptible to infect the children. Mixed infection was more likely to occur in children up to six. The body temperature with human parainfluenza 3 virus infection was lower than 39℃, while the body temperature of human metapneumovirus infection was higher than 39℃. Conclusion Enterovirus, respiratory syncytial virus and parainfluenza virus are the commonly-seen virus pathogens of upper respiratory tract infection in Guiyang area. Multiple PCR detection of respiratory pathogens can be used to provide more accurate medical strategies in the early diagnosis of fever patients.
