摘　　要：目的:研究经肛门改良Soave巨结肠根治术远期并发症对患儿肛门功能及生活质量的影响。方法:选取本院2015年3月-2020年3月行经肛门改良Soave巨结肠根治术的先天性巨结肠(HD)患儿60例,观察随访3、6、12、18个月的并发症发生情况,评价患儿术后肛门功能和生活质量,并对伴随并发症患儿术后肛门功能与生活质量进行相关性分析。结果:随访3、6、12、18个月,患儿并发症发生率分别为16.7%、25.0%、43.3%、46.7%;随访3、6、12、18个月,并发症组患儿的Heikkinen评分和PedsQL4.0评分均显著低于无并发症组(P<0.05);随访12、18个月有并发症组患儿的Heikkinen评分和PedsQL4.0评分均存在显著正相关关系(r=0.888 0、0.799 5,P<0.05)。结论:经肛门改良Soave巨结肠根治术远期并发症对患儿肛门功能及生活质量有明显影响,患儿肛门功能与生活质量间存在明显正相关。Objective:To study the influence of long-term complications of modified Soave Hirschsprung’s disease on anal function and quality of life in children.Method:A total of 60 children with Hirschsprung’s disease (HD) who underwent modified transanal Soave radical operation from March 2015 to March 2020 were selected.After 3,6,12 and 18 months of follow-up,the long-term complications were observed,the anal function and quality of life were evaluated,and the correlation between postoperative anal function and quality of life in children with complications was analyzed.Result:After 3,6,12 and 18 months of follow-up,the incidence of complications in children were 16.7%,25.0%,43.3% and 46.7% respectively;after 3,6,12 and 18 months of follow-up,the Heikkinen scores and PedsQL4.0 scores in the complication group were significantly lower than those in the non complication group (P<0.05);there were significant positive correlations between Heikkinen scores and PedsQL4.0 scores in the complication group at 12 and 18 months of follow-up (r=0.888 0,0.799 5,P<0.05).Conclusion:The long-term complications of modified Soave megacolon radical resection through anus have a significant impact on the anus function and quality of life of the children,and there is a significant positive correlation between anus function and quality of life of the children.
