早孕期绒毛膜隆起的超声诊断及临床意义

摘　　要：目的探讨早孕期绒毛膜隆起的超声声像图特征及其与妊娠结局的相关性。方法回顾性分析在我院行早期妊娠检查的40621例孕妇,其中绒毛膜隆起(chorionic bumps,CB)44例,为CB组;随机抽取绒毛膜下血肿(subchorionic hematoma,SCH)310例,为SCH组,正常早孕334例,为正常组。对三组的自然流产率进行对照分析,并总结绒毛膜隆起的超声图像,如隆起的数目和大小,研究其对妊娠结局的影响及转归。结果 CB组44例孕妇中自然流产16例,自然流产率为36.36%,均高于SCH组和正常组,差异有统计学意义(P<0.05)。CB组按病灶数量分为多发和单发两亚组,自然流产率分别为55.56%、23.08%,两组比较差异有统计学意义(P=0.039)。28例宫内活胎中有20例绒毛膜隆起于后期逐渐消失。结论超声对早孕期绒毛膜隆起的诊断有一定临床价值,密切观察随诊有助于了解病灶的变化及胚胎发育情况。Objective To explore the ultrasonic sonographic characteristics of chorionic bumps during early pregnancy and its correlation with pregnancy outcome. Methods A retrospective analysis of 40621 pregnant women who underwent early pregnancy examinations in our hospital, including 44 cases of chorionic bump(CB group), randomly selected 310 cases of subchorionic hematoma(SCH group), 334 cases of normal early pregnancy(normal group), three the spontaneous abortion rate of the group was compared and analyzed, and the ultrasound images of the chorionic bump were summarized,such as the number and size of the bump, to study its influence on the pregnancy outcome. Results There were 16 spontaneous abortions among the 44 pregnant women in the CB group. The spontaneous abortion rate was 36.36%, which was higher than that of the SCH group and the normal group, and the difference was statistically significant(P<0.05). According to the number of lesions, the CB group was divided into multiple and single subgroups. The spontaneous abortion rates were 55.56% and 23.08%, the difference between the two groups was statistically significant(P=0.039). In 28 cases of live fetuses, 20 cases of chorionic bump disappeared gradually in the later period. Conclusion Ultrasound has a certain clinical value in the diagnosis of chorionic bumps during early pregnancy. Close observation and follow-up help to understand the changes of the lesions and embryonic development.
