摘 要:目的调查分析2017—2019年延安地区新生儿肺透明膜病(NRDS)的临床资料,并对其进行SP-B遗传缺陷研究。方法 138例住院NRDS患儿资料与138例非NRDS患儿(对照组)资料进行对比,并根据NRDS患儿转归情况将死亡患儿(NRDS死亡组)与同数量本院其他原因死亡新生儿(死亡对照组)进行对比分析,研究SP-B遗传缺陷与NRDS关系。结果延安地区138例NRDS患儿主要以男性、早产、Apgar评分<7分、低出生体重、单胎、剖宫产为主;二元Logistic多因素回归分析显示,性别不是NRDS患儿的相关影响(P>0.05),胎龄、Apgar评分、出生体重、孕妇年龄、胎数、高危妊娠、分娩方式、胎粪污染均为NRDS患儿的相关影响因素(P<0.05);138例患儿合并并发症111例(80.43%),经治疗后死亡23例(16.67%);NRDS死亡患儿发生SP-B蛋白缺陷概率大于死亡对照组,且NRDS死亡患儿与SP-B蛋白缺陷呈正相关(r=0.497,P<0.05)。结论 2017—2019年延安地区新生儿肺透明膜病患儿死亡率较高,其影响因素较多,另SP-B遗传缺陷与NRDS病发相关密切,临床可针对SP-B遗传缺陷研究寻找NRDS治疗措施。Objective To investigate and analyze the clinical data of hyaline membrane disease of newborn in Yan’an area from 2017 to 2019, and to study the genetic defect of SP-B. Methods The data of 138 hospitalized NRDS children were compared with the data of 138 non-NRDS children(control group), and the number of children who died(NRDS death group)and the same number of newborns who died from other causes in the hospital were compared according to the outcome of NRDS children(death control group) were compared and analyzed to study the relationship between SP-B genetic defects and NRDS. Results The 138 children with NRDS in Yan’an were mainly male, premature birth, Apgar score <7, low birth weight,single birth, and caesarean section;the binary logistic multivariate regression analysis showed that gender is not related to NRDS children(P>0.05), gestational age, Apgar score, birth weight, maternal age, number of fetuses, high-risk pregnancy,delivery method, and meconium contamination are all related influencing factors in children with NRDS(P<0.05);138 cases of children with complications 111 cases(80.43%) of the disease, 23 cases(16.67%) died after treatment;the probability of SP-B protein deficiency in children with NRDS death was greater than that in the death control group, and the children with NRDS death were positively correlated with SP-B protein deficiency(r=0.497, P<0.05). Conclusion The mortality rate of children with neonatal respiratory distress syndrome in Yan’an from 2017 to 2019 is relatively high, and there are many influencing factors. In addition, SP-B genetic defects are closely related to the onset of NRDS, and clinical studies on SP-B genetic defects can be targeted Look for NRDS treatment measures.
