齐刺电针程氏运感穴区改善中风后遗症期患者肢体运动功能的疗效及对血清BDNF、PDGF的影响

摘　　要：目的:通过齐刺电针程氏运感穴区,观察中风后遗症期患者血清血小板衍生生长因子(PDGF)、脑源性神经营养因子(BDNF)的表达变化及肢体运动功能障碍的临床改善情况。方法:选取2020年1月—2021年1月于黑龙江中医药大学附属二院诊治的中风病后遗症患者60例,并应用随机数字表法将其均分为治疗组和对照组,各30例。对照组采用针刺患侧肢体结合康复训练,治疗组采用齐刺电针程氏运感穴区及患侧肢体结合康复训练,治疗时间为4周。对两组患者进行PDGF、BDNF水平检测,并观察美国国立卫生研究院卒中量表NIHSS评分、运动功能Fugl-Meyer评分量表(FMA)、Barthel(ADL)指数评分以及总有效率变化。结果:经过4周治疗后,治疗组的总有效率为86.67%(26/30),明显高于对照组的总有效率60.00%(18/30),差异具有统计学意义(P<0.05);与治疗前比,治疗后两组患者NIHSS评分明显降低,且治疗组患者评分低于对照组,差异有统计学意义(P<0.05);与治疗前比,治疗后两组患者FMA、Barthel评分明显升高,且治疗组评分明显高于对照组,差异有统计学意义(P<0.05);两组患者BDNF、PDGF水平较治疗前显著升高,且治疗组BDNF、PDGF水平高于对照组,差异具有统计学意义(P<0.05)。结论:齐刺电针程氏运感穴区改善了中风后遗症期患者神经功能及肢体运动功能且临床效果显著,对患者日常生活活动能力有显著提升,并可促进BDNF、PDGF的分泌,具有临床实践应用价值。Objective:To observe the expression changes of platelet-derived growth factor(PDGF)and brain-derived neurotrophic factor(BDNF)in serum and the clinical improvement of limb motor dysfunction in patients with sequelae of stroke by triple electro-needling Cheng's motor sensation area.Methods:60 patients with stroke sequelae were randomly divided into the treatment group and the control group,with 30 cases in each group.The control group was treated with needling the affected limb combined with rehabilitation training;on which basis,the treatment group was also treated with triple electro-needling Cheng's motor sensation area.After 4 weeks of the treatment,the levels of PDGF and BDNF were detected and the changes of the scores of NIHSS,motor function,FMA and Barthel index were observed in the two groups.The total effective rate was also compared between the two groups.Results:After 4 weeks of the treatment,the total effective rate of the treatment group was 86.67%(26/30),which was significantly higher than 60.00%(18/30)of the control group(P<0.05).The scores of NIHSS were significantly decreased after the treatment compared to those before the treatment in the two groups;the decreases in the treatment group were more significant(P<0.05).The scores of FMA and Barthel index were significantly increased after the treatment compared to those before the treatment in the two groups;the increases in the treatment group were more significant(P<0.05).The levels of BDNF and PDGF were higher after the treatment than those before the treatment in the two groups,of which the treatment group was higher than the control group(P<0.05).Conclusion:Triple electro-needling Cheng's motor acupoint area can improve neurological function and limb motor function of the patients with stroke sequela,with its significant clinical effect.It can also significantly improve the activities of daily living of the patients and can promote the secretion of BDNF and PDGF.The therapy has a significant clinical application value.
