软组织补片在手部皮肤软组织缺损修复中的效果分析

摘　　要：目的探讨新型再生可降解生物材料软组织修补片(Porcine Small Intestinal Submucosa,SIS)在修复手部皮肤软组织缺损中的治疗效果与方法。方法2018年6月-2019年6月随机将26例手部皮肤软组织缺损患者分成两组,每组各13例。A组采用SIS补片治疗,B组采用自体取皮植皮治疗。对比两种治疗方法在皮肤缺损愈合中的效果,并记录术后第14天、21天、28天、2个月、4个月后创面区愈合及瘢痕生长情况,长期随访评估创面区愈合后的外观、色泽、弹性、感觉恢复情况,综合评估治疗效果。结果A、B两组创面区均完全愈合,外观色泽接近,皮肤弹性及感觉恢复良好。与B组相比,A组具有无缝线瘢痕、无需取皮植皮、不牺牲供区的优点,能有效促进肌肉外露区的肉芽组织生长,并再生新的表皮组织。结论SIS补片是由猪胶原蛋白组合排列而成的脱细胞异种植入物,具有细胞外基质天然的胶原框架和附加因子,能快速、有效地刺激机体再生表皮组织,并且新生的表皮经过生长与周围皮肤颜色无明显差异,无明显瘢痕增生出现,是一种浅表皮肤软组织缺损的理想修复材料。Objective To explore the effect and methods of a new type of renewable biodegradable soft tissue in the treatment of repairing hand skin and soft tissue.Methods From June 2018 to June 2019,we randomly divided 26 patients with hand skin and soft tissue defects into 2 treatment groups,each with 13 Patients:Group A was treated with SIS patch,and group B was treated with autologous skin grafting.Compared the effects of two treatments on the skin defect healing,and recorded the wound healing and scar growth on the 14 d,21 d,28 d,and 2 months,4 months after the operation,long-term followed-up to evaluate the appearance,color,elasticity,and sensory recovery of the wound area after healing,and comprehensively evaluated the treatment effect.Results The wound areas of two groups were completely healed,the appearance and color of scars were similar,and the skin elasticity and sensation were restored.Compared with group B,group A had the advantages of sutureless scar,no skin grafting,no sacrifice of the donor site,and can effectively promote granulation in the exposed area of the muscle,so the tissue growed and regenerated new epidermal tissue.Conclusion The SIS repair patch is an acellular heterogeneous patch composed of a combination of porcine collagen.This kind of implant has the natural collagen framework of the extracellular matrix and additional factors,which can quickly and effectively stimulate the body to regenerate the epidermis tissue,and the new epidermis has grown up with no obvious difference in color from the surrounding skin,and no obvious scar hyperplasia.It is a kind of an ideal repair material for superficial skin and soft tissue defects.
