摘　　要：目的探究血清α-L-岩藻糖苷酶(AFU)、糖类抗原724(CA724)和人附睾蛋白4(HE4)及其联合检测在妊娠合并肿瘤疾病中的诊断价值。方法选取2018年1月至2020年9月于本院接受正常孕检的孕妇300例,根据是否合并肿瘤疾病分为合并肿瘤妊娠组(n=111)和正常妊娠组(n=189),另选取同期本院体检的未妊娠健康女职工100例为对照组。比较3组的血清AFU、CA724和HE4水平,分析不同孕期合并肿瘤妊娠组和正常妊娠组血清AFU、CA724和HE4水平,探讨血清AFU、CA724和HE4水平变化对判断良恶性肿瘤的作用;受试者工作特征曲线(ROC)评估AFU、CA724、HE4及其联合检测在妊娠合并肿瘤疾病中的诊断价值。结果3组的血清AFU、HE4水平比较,合并肿瘤妊娠组>正常妊娠组>对照组(P<0.05);合并肿瘤妊娠组的血清CA724水平高于正常妊娠组和对照组(P<0.05),正常妊娠组和对照组的血清CA724水平比较,差异无统计学意义(P>0.05)。合并肿瘤妊娠组不同孕期血清AFU、CA724和HE4水平比较存在明显差异,正常妊娠组不同孕期血清AFU水平比较存在明显差异(P<0.05);妊娠早期、妊娠中期和妊娠晚期,合并肿瘤妊娠组的血清AFU、CA724和HE4水平均高于正常妊娠组(P<0.05)。良性卵巢疾病孕妇的血清AFU、CA724和HE4水平明显低于恶性卵巢肿瘤孕妇(P<0.05)。ROC曲线分析结果显示,血清AFU、CA724和HE4联合诊断妊娠合并肿瘤疾病及妊娠合并恶性肿瘤的AUC值均明显高于单个指标检测(P<0.05)。结论妊娠合并肿瘤疾病孕妇的血清AFU、CA724和HE4水平较健康孕妇、健康未妊娠者高,血清AFU、CA724和HE4水平在妊娠合并肿瘤患者中随着孕期的增加其水平明显升高;妊娠合并恶性肿瘤患者较合并良性肿瘤患者明显升高,三者联合检测对于妊娠合并良恶性肿瘤具有较高的诊断价值。Objective To explore the diagnostic values of serumα-L-fucosidase(AFU),carbohydrate antigen 724(CA724),human epididymal protein 4(HE4)and their joint detection in tumors during pregnancy.Methods Three hundred pregnant women who received normal pregnancy test in the hospital were enrolled in this study between January,2018 and September,2020.According to the presence or absence of tumor,these pregnant women were divided into the pregnancy with tumor group(n=111)and normal pregnancy group(n=189),while 100 healthy females who were not pregnant were selected as the control group.Levels of serum AFU,CA724 and HE4 in all three groups were compared.Levels of serum AFU,CA724 and HE4 in the pregnancy with tumor group during different pregnancy stages and in the normal pregnancy group were analyzed.The role of changes in serum AFU,CA724 and HE4 levels in judging benign and malignant tumors was explored.Receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic values of AFU,CA724,HE4 and their joint detection in pregnancy with tumors.Results Serum AFU and HE4 levels of the pregnancy with tumor group,the normal pregnancy group,and the control group decreased in order(P<0.05).Serum CA724 level of the pregnancy with tumor group was higher than that of the normal pregnancy group and the control group(P<0.05),but with no significant difference between the normal pregnancy group and the control group(P>0.05).There were significant differences in serum AFU,CA724 and HE4 levels in the pregnancy with tumor group during different pregnancy stages,and there were significant differences in serum AFU level in the normal pregnancy group during different pregnancy stages(P<0.05).Serum AFU,CA724 and HE4 levels in the pregnancy with tumor group during different pregnancy stages were all higher than those in the normal pregnancy group(P<0.05).Serum AFU,CA724 and HE4 levels of pregnant women with benign ovarian disease were significantly lower than those with malignant ovarian tumors(P<0.05).ROC curve analysis showed t
