摘 要:目的探讨血清脂蛋白相关磷脂酶A2(Lp-PLA2)与高敏C-反应蛋白(hs-CRP)联合检测在冠状动脉粥样硬化性心脏病中的诊断价值。方法于2019年5月至2021年5月,选取体检中心参加体检的患者380例,根据其既往史和就诊信息中提示是否患有冠状动脉粥样硬化性心脏病,将患者分为对照组(C组,n=305)和冠状动脉粥样硬化组(G组,n=75)。对比两组患者一般资料和实验室检查指标,并采用Logistic回归模型分析影响冠状动脉粥样硬化性心脏病的危险因素,通过ROC曲线分析Lp-PLA2、hs-CRP及二者联合检测对冠状动脉粥样硬化性心脏病的预测价值。结果与C组比较,G组患者年龄、BMI、高血压患病率、TC、TG、Lp-PLA2及hs-CRP水平升高,差异有统计学意义。逻辑回归分析结果显示,年龄(OR=4.01,P=0.006),BMI(OR=3.92,P=0.004),高血压患病率(OR=3.76,P=0.013),Lp-PLA2(OR=3.87,P=0.016)及hs-CRP(OR=3.61,P=0.023)水平是冠心病发病的独立危险因素。ROC曲线分析显示,Lp-PLA2联合hs-CRP检测的AUC值高于Lp-PLA2及hs-CRP单项检测。结论Lp-PLA2及hs-CRP是冠心病发病的独立危险因素,Lp-PLA2联合hs-CRP对于冠心病的诊断价值优于Lp-PLA2或hs-CRP。Objective The aim of this study was to investigate the diagnostic value of Lp-PLA2 combined hs-CRP in coronary atherosclerotic heart disease.Methods From May,2019 to May,2021,380 patients who participated in the physical examination in the Physical Examination Center were selected according to whether they had coronary atherosclerotic heart disease based on their medical history.These patients were divided into the control group(Group C,n=305)and coronary atherosclerosis group(group G,n=75).The general information and laboratory examination indexes of the two groups were compared.Logistic regression model was applied to analyze the risk factors of coronary atherosclerotic heart disease.ROC curve was drawn to analyze the predictive value of Lp-PLA2,hs-CRP and their combined detection for coronary atherosclerotic heart disease.Results Compared with group C,the levels of age,BMI,incidence of hypertension,TC,TG,Lp-PLA2 and hs-CRP in group G were significantly higher.Logistic regression analysis showed that age(OR=4.01,P=0.006),BMI(OR=3.92,P=0.004),incidence of hypertension(OR=3.76,P=0.013),Lp-PLA2(OR=3.87,P=0.016)and hs-CRP(OR=3.61,P=0.023)were independent risk factors for coronary heart disease.ROC curve analysis showed that AUC value of Lp-PLA2 combined with hs-CRP was higher than that of Lp-PLA2 and hs-CRP individually.Conclusion Lp-PLA2 and hs-CRP are independent risk factors of coronary heart disease.The diagnostic value of Lp-PLA2 and hs-CRP in the diagnosis of coronary heart disease is better than that of Lp-PLA2 or hs-CRP individually.
