大学生腰围-三酰甘油指数与胰岛素抵抗的相关性及识别代谢综合征的准确性分析

摘　　要：目的本研究探讨大学生腰围-三酰甘油指数(WT)与胰岛素抵抗的相关性,并探讨腰围三酰甘油指数(WT指数)识别代谢综合征准确性。方法选取448名18~22岁大学生的横断面调查。计算WT指数=腰围(cm)×三酰甘油(mmol/L),胰岛素抵抗指数(HOMA-IR)=(真胰岛素TI×空腹血糖FPG)/22.5。根据WI指数三分位数分为A组(下三分位数组),B组(中三分位数组)和C组(上三分位数组),比较3组胰岛素抵抗情况,分析WT指数与HOMA-IR的相关性,并分析男、女性大学生WT指数识别代谢综合征的准确性。结果B组和C组平均胰岛素抵抗指数(HOMA-IR)水平均高于A组(A组1.15(0.93~1.64),B组1.36(1.02~1.83),C组1.51(1.14~2.11),P<0.05)。WT指数与HOMA-IR呈正相关(r=0.210,P=0.000)。WT指数识别代谢综合征的ROC曲线下面积分别为男生0.910(95%CI:0.751~1.069,P=0.002)和女生0.857(95%CI:0.657~1.057,P=0.014)。男生在149.5时诊断指数最大,灵敏性和特异性分别为80.0%和100.0%,女生在84.5时诊断指数最大,灵敏性和特异性分别为75.0%和92.8%。结论WT指数与大学生胰岛素抵抗和代谢综合征密切相关,WT指数可做为识别大学生代谢综合征的简单工具。Objective To investigate the correlation between waist circumference-triglyceride index(WT index)and insulin resistance as well as its accuracy in diagnosis of metabolic syndrome in university students.Methods A cross-sectional study was oerformed in 448 university students aged 18~22 years in Qinhuangdao city.WT index was calculated by waist circumference(cm)×triglyceride(mmol/L),and insulin resistance index(HOMA-IR)calculated by true insulin×fasting blood Glucose/22.5.According to the tri-sectional quantiles of WT index,these students were divided into three groups,group A(down quantiles),group B(middle quantiles)and group C(upper quantiles).The correlation between the WT index and HOMA-IR was analyzed,moreover,the accuracy of the WT index for male and female university students in diagnosis of metabolic syndrome was analyzed.Results The levels of HOMA-IR in group B and group C were significantly higher than those in group A(P<0.05).Moreover WT index was positively correlated with HOMA-IR(r=0.210,P<0.01).The area under the ROC curve of WT index in diagnosis of metabolic syndrome was 0.910 for males(95%CI:0.751~1.069,P<0.01)and 0.857 for females(95%CI:0.657~1.057,P<0.05).When the ROC was 149.5 for male students,the sensitivity and specificity were 80.0%and 100.0%,respectively,when the ROC was 84.5 for female students,the sensitivity and specificity were 75.0%and 92.8%,respectovely.Conclusion The WT index is closely related to insulin resistance and metabolic syndrome in university students,therefor,WT index can be regarded as a simple tool to diagnose the metabolic syndrome in university students.
