基于加速康复外科理念的吴茱萸粗盐热敷联合穴位针灸理疗对结直肠癌术后肠功能恢复的影响研究

摘　　要：目的分析基于加速康复外科(ERAS)理念的吴茱萸粗盐热敷联合穴位针灸理疗对结直肠癌术后患者肠功能恢复效果的影响。方法纳入2020年6月至2020年11月我院收治的296例结直肠癌患者作为研究对象,根据术后康复护理方式的不同,将患者分为四组:对照组(n=72)、吴茱萸粗盐热敷组(n=74)、穴位针灸理疗组(n=77)、吴茱萸粗盐热敷联合穴位针灸理疗组(n=73)。比较四组肠功能恢复情况(术后首次排气时间、排粪时间)、术后腹胀及疼痛评分、护理满意度及患者住院时间和费用。结果吴茱萸粗盐热敷联合穴位针灸理疗组首次排气时间和首次排粪时间均早于对照组(均P<0.05);吴茱萸粗盐热敷联合穴位针灸理疗组的腹胀评分均低于其他三组(均P<0.05);吴茱萸粗盐热敷联合穴位针灸理疗组的疼痛评分高于对照组及吴茱萸粗盐热敷组,但低于穴位针灸理疗组(均P<0.05);吴茱萸粗盐热敷联合穴位针灸理疗组与其他三组之间的护理满意度比较差异均无统计学意义(均P>0.05);四组住院时间及住院费用比较差异均无统计学意义(均P>0.05)。结论基于ERAS理念的吴茱萸粗盐热敷联合穴位针灸理疗能促进患者术后肠功能恢复,减轻患者术后腹胀,对改善患者生活质量有积极意义且不增加住院时间和住院费用,但在缓解术后疼痛、提升护理满意度方面效果不明显。Objectives To investigate the effects of crude salt and Evodia rutaecarpa hot compress combined with acupoint acu⁃puncture and physiotherapy based on the concept of enhanced recovery after surgery(ERAS)on intestinal function recovery for colorectal cancer.Methods Two hundred ninety-six patients with colorectal cancer treated at our hospital between June and No⁃vember 2020 were included.According to their nursing methods,patients were divided into four groups:control group(n=72),Group A(receiving crude salt and Evodia rutaecarpa hot compress,n=74),Group B(receiving acupoint acupuncture,n=77),and Group C(receiving crude salt and Evodia rutaecarpa hot compress combined with acupoint acupuncture and physiotherapy,n=73).Intestinal function recovery(time to first postoperative flatulence,time to first postoperative defecation),scores of postoperative ab⁃dominal distension and pain,patients’satisfaction on nursing services,length of stay,and hospitalization expenses were compared among the four groups.Results Group C had significantly shorter time to first postoperative flatulence and time to first postoper⁃ative defecation than the control group(P<0.05).Group C had significantly lower score of abdominal distension than the other three groups(P<0.05).Group C had significantly higher pain score than the control group and Group A but lower pain score than Group B(P<0.05).Patients’satisfaction with nursing services in Group C did not differ significantly from that in the other three groups(P>0.05).Length of stay and hospital expenses were similar among the four groups(P>0.05).Conclusion For colorectal cancer patients,crude salt and Evodia rutaecarpa hot compress combined with acupoint acupuncture and physiotherapy based on the concept of ERAS can promote intestinal function recovery,alleviate postoperative abdominal distension,improve pa⁃tients’quality of life.At the same time,it does not increase patients’length of stay and hospitalization expenses.However,it does not significantly alleviate postoperativ
