弹力袜加压与交通静脉离断术治疗下肢静脉性溃疡的疗效比较

摘　　要：目的比较弹力袜加压与交通静脉离断术治疗下肢静脉性溃疡(VLU)的疗效。方法将108例VLU患者随机分为对照组和观察组,每组54例。对照组患者采用交通静脉离断术治疗,观察组患者采用弹力袜加压治疗。比较两组患者治疗前后的创面积分、疼痛视觉模拟量表(VAS)评分、治疗28 d时的疗效,治疗后6个月内溃疡愈合及并发症情况,治疗3个月、6个月时患肢小腿周径缩小值及出院后24个月内溃疡复发情况。结果治疗后,两组患者的创面积分、VAS疼痛评分均降低,且观察组患者的创面积分、VAS评分下降更明显(均P<0.05)。观察组患者的临床总有效率及溃疡愈合率均高于对照组,治疗3个月及6个月时观察组患肢小腿周径缩小值均大于对照组,并发症发生率及溃疡复发率均低于对照组(均P<0.05)。结论相较于交通静脉离断术,弹力袜加压治疗VLU能更有效地减轻患肢疼痛和肿胀程度,减少并发症发生及溃疡复发,治疗效果更好。Objective To compare the efficacy of compression with elastic stockings versus disconnection of communicating vein for the treatment of venous leg ulcer(VLU).Methods A total of 108 VLU patients were randomly divided into control group and observation group,with 54 cases in each group.Patients in the control group were treated with disconnection of communicating vein,and patients in the observation group received compression therapy with elastic stockings.The related indices were compared between the two groups,including the wound score and the Visual Analogue Scale(VAS)pain score before and after the treatment,the efficacy on day 28 of the treatment,the condition of ulcer healing and the incidence of complications within six months after the treatment,the peripheral diameter reductions of affected leg after three and six months of the treatment,as well as the incidence of ulcer recurrence within 24 months after discharge.Results After the treatment,the wound score and VAS pain score were decreased in both groups,and more significant reductions were found in the wound score and VAS pain score of patients in the observation group(all P<0.05).The total clinical effective rate and the ulcer healing rate of patients in the observation group were higher than those in the control group,and after three and six months of the treatment,the observation group exhibited an increased peripheral diameter reduction of the affected leg,a lower incidence rate of complications,and a decreased ulcer recurrence rate as compared with the control group(all P<0.05).Conclusion Compared with disconnection of communicating vein,compression therapy with elastic stockings can reduce the severity of pain and swelling of affected leg more effectively,diminish the incidence of complications and ulcer recurrence,and achieve better treatment effects in VLU.
