纳布啡静推复合右美拖咪定滴鼻用于小儿无痛纤支镜检查中镇静的作用及分析

摘　　要：目的探讨对无痛纤支镜检查患儿采用纳布啡静推复合右美托咪定滴鼻进行麻醉所起到的镇静效果。方法回顾性分析南阳医学高等专科学校第一附属医院2019年11月至2021年3月接受无痛纤支镜检查的90例患儿,采用随机数字表法将其分为两组,对照组采用右美托咪定麻醉,在此基础上,观察组加用纳布啡进行麻醉,比较两组患儿接受不同方案麻醉后各时间点的疼痛程度、镇静效果、生命体征情况及发生不良反应的情况。结果与对照组比较,观察组在术后即刻、术后6 h、术后24 h等时间点的疼痛程度更低,所获镇静效果更高(P<0.05);两组患儿在接受检查前的生命体征情况比较,差异无统计学意义(P>0.05),在检查过程中及检查结束后,观察组较对照组HR及SBP水平更低(P<0.05);观察组发生不良反应的概率更低(P<0.05)。结论对无痛纤支镜检查患儿采用纳布啡静推复合右美托咪定滴鼻进行麻醉可以有效降低疼痛程度,提高镇静效果,且不良反应较少,具有较高的临床推广价值。【Objective】To investigate and analyze the sedative effect of intravenous nalbuphine combined with dexmedetomidine nasal drops on children undergoing painless fiberoptic bronchoscopy.【Methods】Ninety children undergoing painless fiberoptic bronchoscopy in our hospital from November 2019 to March 2021 were selected and randomly divided into two groups.The control group was anesthetized with dexmedetomidine.On this basis,the observation group was anesthetized with nalbuphine.The degree of pain,sedative effect,vital signs and adverse reactions were compared between the two groups at each time point after receiving different anesthesia schemes.【Results】Compared with the control group,the pain degree of the observation group was significantly lower immediately after operation,6 hours after operation and 24 hours after operation,and the sedative effect was significantly higher(P<0.05).There was no significant difference in vital signs between the two groups before examination(P>0.05).During and after examination,HR and SBP levels of the observation group were significantly lower than those of the control group(P<0.05).The probability of adverse reactions in the observation group was significantly lower(P<0.05).【Conclusion】For children undergoing painless fiberoptic bronchoscopy,intravenous injection of nabuprofen combined with nasal drip of dexmedetomidine can effectively reduce the degree of pain,improve the sedative effect,and has less adverse reactions,which has high clinical promotion value.
