针刺八髎穴联合功能康复训练治疗脑卒中后神经源性膀胱尿潴留临床观察

摘　　要：目的:观察针刺八髎穴联合功能康复训练治疗脑卒中后神经源性膀胱(PSNB)尿潴留的临床效果。方法:56例按随机数字表法分为两组各28例。两组均行功能康复训练,观察组加用针刺八髎穴治疗。结果:总有效率观察组92.86%,对照组71.43%,两组比较差异有统计学意义(P<0.05)。治疗后日均排尿次数、日均单次排尿量观察组分别为(5.63±1.12)次、(236.52±10.52)mL,对照组分别为(4.12±0.83)次、(211.39±10.24)m L,两组比较差异有统计学意义(P<0.05)。治疗后残余尿量观察组为(62.35±7.15)m L,对照组为(82.24±9.6)m L;膀胱最大容量、最大尿流速率观察组为(365.52±25.19)m L、(15.39±2.28)m L/s,对照组(314.21±23.0)m L、(12.48±2.0)m L/s,两组比较差异有统计学意义(P<0.05)。结论:针刺八髎穴联合功能康复训练可增强治疗效果,提高最大尿流速率,增加单次排尿量,减少膀胱残余尿量,促进康复。Objective:To observe the clinical effect of acupuncture at Baliao acupoints combined with functional rehabilitation training in the treatment of post-stroke neurogenic bladder(PSNB) urinary retention. Methods: Fifty-six cases were divided into two groups, with 28 cases in each group, according to the random number table method. Both groups received functional rehabilitation training, and the observation group were treated with acupuncture at Baliao acupoints in addition. Results: The total effective rate was 92.86% in the observation group and 71.43% in the control group. The difference between the two groups was statistically significant(P<0.05). After treatment, the average daily urination times and daily average single urination volume were(5.63±1.12) times and(236.52±10.52) mL in the observation group, and(4.12±0.83) times and(211.39±10.24) mL in the control group, respectively. The difference between the groups was statistically significant(P<0.05). After treatment, the residual urine volume was(62.35±7.15) mL in the observation group,while(82.24±9.6) mL in the control group. The maximum bladder capacity and maximum urine flow rate of the observation group were(365.52±25.19) mL and(15.39±2.28) mL/s respectively while those of the control group were(314.21±23.0)mL and(12.48±2.0) mL/s respectively. The difference between the two groups was statistically significant(P<0.05).Conclusion: Acupuncture at Baliao acupoints combined with functional rehabilitation training can enhance the therapeutic effect, increase the maximum urinary flow rate, increase the single urinary volume, reduce the residual urine volume of the bladder, and promote recovery.
