前外侧小切口联合L型锁定钢板治疗胫骨远端骨折的疗效观察

摘　　要：目的 分析胫骨远端骨折疾病特点,评价前外侧小切口联合L型锁定钢板治疗的临床优势。方法 84例胫骨远端骨折患者,采用随机数字表法分为对照组与观察组,每组42例。对照组采取切开复位内固定术治疗,观察组采取前外侧小切口+L型锁定钢板治疗。比较两组围术期指标、术后并发症发生情况以及踝关节功能恢复效果。结果 观察组踝关节功能恢复优良率95.24%高于对照组的73.81%,并发症发生率2.38%低于对照组的16.67%,差异有统计学意义(P<0.05)。观察组手术时间(58.50±6.50)min、术后住院时间(6.50±1.50)d、骨痂生成时间(36.50±3.50)d、骨折愈合时间(185.50±10.50)d短于对照组的(103.50±8.50)min、(11.50±2.50)d、(42.50±4.50)d、(240.50±15.50)d,术中出血量(63.50±8.50)ml少于对照组的(173.50±9.80)ml,差异有统计学意义(P<0.05)。结论 前外侧小切口与L型锁定钢板联合治疗胫骨远端骨折手术损伤小、术后恢复快,具有安全性、有效性价值。Objective To analyze the characteristics of distal tibial fractures and evaluate the clinical advantages of anterolateral min-incision combined with L-shaped locking plate treatment.Methods A total of 84 patients with distal tibial fractures were divided in the control group and the observation group according to random numerical table.The control group was treated with open reduction internal fixation,and the observation group was treated with anterolateral min-incision and L-shaped locking plate.The perioperative indicators,postoperative complications,and recovery effect of ankle joint function were compared between the two groups.Results The good recovery rate of ankle joint function 95.24%in the observation group was higher than 73.81% in the control group,and the complication rate 2.38% was lower than 16.67% in the control group.All the differences were statistically significant(P<0.05).The operation time(58.50±6.50)min,postoperative hospitalization time(6.50±1.50)d,callus formation time(36.50±3.50)d,and fracture healing time(185.50±10.50)d of the observation group were shorter than(103.50±8.50)min,(11.50±2.50)d,(42.50±4.50)d,(240.50±15.50)d of the control group;the intraoperative blood loss(63.50±8.50)ml of the observation group was less than(173.50±9.80)ml of the control group,and the difference was statistically significant(P<0.05).Conclusion The combination of anterolateral min-incision and L-shaped locking plate for the treatment of distal tibial fractures has safety and effectiveness value with less surgical injury and faster postoperative recovery.
