多巴胺联合硝普钠治疗心梗后左心衰的临床效果

摘　　要：目的 分析心肌梗死(心梗)后左心衰竭(左心衰)应用多巴胺联合硝普钠治疗的效果及安全性。方法 50例心梗后左心衰患者,按照不同的治疗方式不同将其分为对照组和研究组,每组25例。对照组患者行常规治疗,研究组患者行多巴胺联合硝普钠治疗。比较两组患者治疗后的心功能指标[左心室舒张末期内径(LVEDD)、左室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室短轴缩短率(FS)]以及治疗效果。结果 研究组患者治疗后的LVEDD(42.67±5.13)mm、LVESD(27.45±1.18)mm均低于对照组的(51.28±3.84)、(32.52±0.96)mm, LVEF(56.85±2.27)%、FS(28.46±1.52)%均高于对照组的(47.56±3.09)、(25.59±0.94)%,差异均具有统计学意义(P<0.05)。对照组治疗7例显效, 11例有效, 7例无效,治疗总有效率为72.00%;研究组治疗10例显效, 14例有效, 1例无效,治疗总有效率为96.00%。研究组治疗总有效率高于对照组,差异具有统计学意义(χ^(2)=5.357, P<0.05)。结论 采用多巴胺联合硝普钠治疗心梗后左心衰,能够有效改善患者的心功能,纠正其临床相关症状,同时还能确保治疗安全性,促进患者的康复和预后。Objective To analyze the effect and safety of combined therapy of dopamine and sodium nitroprusside in the treatment of left heart failure after myocardial infarction.Methods A total of 50 patients with left heart failure after myocardial infarction were divided into the control group and the research group according to the difference of drug intervention,with 25 cases in each group.Patients in the control group received conventional treatment,and patients in the research group received dopamine in combination with sodium nitroprusside.Both groups were compared in terms of cardiac function indexes[left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD)and fraction shortening(FS)]after treatment,and therapeutic effect.Results After treatment,the LVEDD(42.67±5.13)mm and LVESD(27.45±1.18)mm of the research group were lower than(51.28±3.84)and(32.52±0.96)mm of the control group,the LVEF(56.85±2.27)%and FS(28.46±1.52)%were higher than(47.56±3.09)and(25.59±0.94)%of the control group;all the differences were statistically significant(P<0.05).In the control group,7 cases were markedly effective,11 cases were effective,and 7 cases were ineffective,with total effective rate of treatment as 72.00%;in the research group,10 cases were markedly effective,14 cases were effective,and 1 case was ineffective,with total effective rate of treatment as 96.00%.The total effective rate of treatment in the research group was higher than that in the control group,and the difference was statistically significant(χ2=5.357,P<0.05).Conclusion Dopamine in combination with sodium nitroprusside for the treatment of left heart failure after myocardial infarction can effectively improve the cardiac function,correct its clinically relevant symptoms,and at the same time ensure the safety of treatment,and promote the recovery and prognosis of patients.
