血府逐瘀汤联合低分子肝素钙防治门静脉高压症断流术后门静脉血栓的效果

摘　　要：目的 探究血府逐瘀汤联合低分子肝素钙防治门静脉高压症断流术后门静脉血栓的效果。方法 78例门静脉高压症断流术患者,采用随机数字表法分为参照组和治疗组,每组39例。参照组给予低分子肝素钙治疗,治疗组给予血府逐瘀汤联合低分子肝素钙治疗。对比两组治疗前后凝血指标水平、门静脉血栓以及其他并发症发生情况。结果 治疗后,治疗组凝血酶原时间(12.32±0.67)s、凝血酶时间(17.03±2.04)s、活化部分凝血活酶时间(33.20±1.40)s长于参照组的(11.74±0.80)、(15.30±2.17)、(30.13±1.34)s,血小板计数(275.20±3.32)×10^(9)/L低于参照组的(282.32±2.95)×10^(9)/L,差异具有统计学意义(P<0.05)。治疗组门静脉血栓发生率5.13%低于参照组的23.08%,差异具有统计学意义(P<0.05);两组其他并发症发生率对比差异无统计学意义(P>0.05)。结论 血府逐瘀汤联合低分子肝素钙用于门静脉高压症断流术患者中可改善凝血功能,更有效预防门静脉血栓,用药安全性较理想,可优先选择。Objective To investigate the effect of Xuefu Zhuyu Decoction combined with low-molecularweight heparin calcium in the prevention and treatment of portal vein thrombosis after devascularization for portal hypertension.Methods A total of 78 patients with portal hypertension undergoing devascularization were divided into the reference group and the treatment group by random numerical table,with 39 cases in each group.The reference group was treated with low-molecular-weight heparin calcium,and the treatment group was treated with Xuefu Zhuyu Decoction and low-molecular-weight heparin calcium.The coagulation index levels before and after treatment,portal vein thrombosis and other complications were compared between the two groups.Results After treatment,the prothrombin time(12.32±0.67)s,thrombin time(17.03±2.04)s,and activated partial thromboplastin time(33.20±1.40)s in the treatment group were longer than(11.74±0.80),(15.30±2.17),and(30.13±1.34)s in the reference group,and the platelet count(275.20±3.32)×10^(9)/L was lower than(282.32±2.95)×10^(9)/L in the reference group.All the differences were statistically significant(P<0.05).The incidence of portal vein thrombosis 5.13% in the treatment group was lower than 23.08% in the reference group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of other complications between the two groups(P>0.05).Conclusion Xuefu Zhuyu Decoction in combination with low-molecular-weight heparin calcium can improve blood coagulation function and prevent portal vein thrombosis more effectively in patients with portal hypertension after devascularization.The drug safety is ideal and can be preferred.
