摘  要:目的评价正常足月新生儿不常规口鼻咽吸引的干预效果。方法检索中英文数据库中关于新生儿分娩后采用口鼻咽吸引和不采用口鼻咽吸引对出生后结局干预效果的随机对照试验,利用RevMan 5.3软件进行统计处理。结果纳入文献13篇,总样本量为6665例,2组采用不同干预后,meta分析结果显示,新生儿复苏发生率(95%CI 0.52~1.17,P=0.23)、NICU转入率(95%CI 0.57~1.07,P=0.13)、1 min Apgar评分>7分占比(95%CI 1.00~1.04,P=0.13)、5 min Apgar评分>7分占比(95%CI 1.00~1.02,P<0.05)、动脉血pH值(95%CI-0.02~0.02,P<0.001)。结论胎儿分娩后不采用口鼻咽吸引进行呼吸道清理,可提高其血氧含量及出生后活力,但在新生儿复苏发生率、NICU转入率的不良预后中不显著,应对正常足月新生儿的呼吸道清理做出正确的判断和处理。Objective To evaluate the intervention effects of unconventional oropharyngeal suction in normal term neonates.Methods The Chinese and English databases were searched,and randomized controlled trials of the effects of interventions on postnatal outcomes were performed with oro-nasopharyngeal aspiration and non-oropharyngeal aspiration after delivery,and statistical analysis was performed using RevMan 5.3 software.Results Thirteen literatures were included,with a total sample size of 6665 cases.After different interventions in the two groups,the meta analysis showed that the incidence of neonatal resuscitation(95%CI 0.52-1.17,P=0.23),NICU transfer rate(95%CI 0.57-1.07,P=0.13),proportion of 1 min Apgar score>7 points(95%CI 1.00-1.04,P=0.13),proportion of 5 min Apgar score>7 points(95%CI 1.00-1.02,P<0.05),pH value of arterial blood(95%CI-0.02-0.02,P<0.001).Conclusion The fetal respiratory tract cleaning without oronasopharynx suction after delivery can improve its blood oxygen content and postnatal vitality,but it is not significant in the adverse prognosis of the incidence of neonatal resuscitation and NICU transfer rate.Correct judgment and treatment should be made for the respiratory tract cleaning of normal full-term newborns.
