乳头刺激联合缩宫素静脉输注引产对羊水偏少孕产妇产程及分娩结局的影响

摘　　要：目的探讨乳头刺激联合缩宫素静脉输注引产对羊水偏少孕产妇产程及分娩结局的影响。方法选择2020年6月—2021年1月东部战区总医院秦淮医疗区产科收治的60例羊水偏少孕产妇作为研究对象,随机分为对照组和研究组,各30例,对照组给予缩宫素静脉输注,研究组在对照组的基础上指导孕产妇进行有效的乳头刺激,密切观察孕产妇临产开始的时间,第一、二产程的时间,胎儿宫内窘迫、新生儿窒息、产后2 h出血发生率,分娩结局及护理满意度。结果研究组引产后临产开始的时间,第一、第二产程时间均短于对照组(P<0.05);研究组胎儿宫内窘迫、新生儿窒息、产后2 h出血发生率低于对照组(P<0.05);研究组初产妇自然分娩率、护理满意度高于对照组(P<0.05)。结论羊水偏少孕产妇通过乳头刺激联合缩宫素静脉输注能达到良好的引产效果,降低了分娩风险,保障了母婴安全。Objective To investigate the effect of nipple stimulation combined with intravenous drip of oxytocin on the labor duration and outcome of pregnant women with oligohydramnios.Methods A total of 60 pregnant women with oligohydramnios who were admitted to the Medical Campus of Qinhuai District,the General Hospital of Eastern Theater Command from June 2020 to January 2021 were enrolled.They were randomly divided into two groups(n=30):a control group and a research group.The control group was injected by intravenous drip with oxytocin,while the research group underwent effective nipple stimulation on the basis of the control group.The onset time of delivery,the first and second stage of labor duration,fetal distress,neonatal asphyxia,the bleeding rate 2 h after delivery,delivery outcome and nursing satisfaction were observed.Results Compared with the control group,the research group presented decreases in the onset time of delivery,the first and second stage of labor duration(P<0.05);decreases in the incidences of fetal distress,neonatal asphyxia and the bleeding rate 2 h after delivery(P<0.05);and increases in the rate of spontaneous delivery and nursing satisfaction(P<0.05).Conclusions For pregnant women with oligohydramnios,nipple stimulation combined with intravenous drip of oxytocin can achieve good effects,reduce the risk of delivery and guarantee the safety of mothers and newborns.
