摘　　要：目的探讨阴道上皮内瘤变(VAIN)的临床特点。方法收集2013年1月至2018年12月在福建医科大学附属闽东医院宫颈疾病门诊进行阴道镜检查并经组织病理学检查确诊的VAIN患者回顾性分析。结果VAIN的病变程度与液基细胞学结果异常程度存在正相关(P<0.001)。随着VAIN分级的增加,HPV16阳性病例的比例相应增加(P<0.05)。VAINⅠ/LSIL、VAINⅡ~Ⅲ/HSIL合并CIN2+及以上的比例分别为63.64%(21/33)、92.50%(37/40)(P<0.05),VAIN病变程度与宫颈病变程度具有一致性(P<0.05)。结论阴道上皮内瘤变的细胞学和HR-HPV检测在VAIN早期发现中非常重要,但联合筛查对VAIN的筛查效能最佳。VAIN的发生与CIN具有明显相关性,可以非常有效地预防发生高级别阴道癌前病变及浸润癌。Objective To explore the clinical features of vaginal intraepithelial neoplasia(VAIN).Methods A retrospective analysis of VAIN patients who underwent colposcopy at the Cervical Disease Clinic of Mindong Hospital Affiliated to Fujian Medical University from January 2013 to December 2018,and confirmed by histopathological examination were collected from January 1,2013 to December 31,2018.Results The degree of VAIN lesions was positively correlated with the abnormal degree of liquid-based cytology(P<0.001).With the increase of VAIN classification,the proportion of HPV16 positive cases increased correspondingly(P<0.05).The ratios of VAINⅠ/LSIL,VAINⅡ-Ⅲ/HSIL combined with CIN2+and above were 63.64%(21/33)and 92.50%(37/40)respectively(P<0.05).The degree of VAIN lesions was consistent with the degree of cervical lesions(P<0.05).Conclusion Cytology and HR-HPV detection of vaginal intraepithelial neoplasia are very important in the early detection of VAIN,but combined screening has the best screening effect for VAIN.The occurrence of VAIN is obviously related to CIN,which can effectively prevent the occurrence of high-grade vaginal precancerous lesions and invasive cancer.
