摘 要:目的探讨有创-无创序贯通气治疗急性有机磷农药中毒(AOPP)中间综合征(IMS)所致呼吸衰竭的疗效。方法选取我院在2019年1月至2021年7月所治疗的IMS患者313例,按照随机分为试验组(149例)和对照组(164例),其中对照组进行有创呼吸机治疗,试验组在对照组的基础上使用有创与无创序贯呼吸机进行治疗,比较两组患者预后和并发症发生情况。结果试验组的治疗总有效率明显高于对照组(P<0.05)。治疗后,两组患者血清胆碱酯酶比较,差异具有统计学意义,P<0.05。两组患者意识恢复时间、自主呼吸恢复时间、胆碱酯酶活力恢复60%的时间、住院时间、总上机时间和有创通气时间比较,试验组的各项恢复时间均短于对照组,差异具有统计学意义,P<0.05。两组患者院内感染、呼吸及相关肺炎和撤机失败率比较,试验组的各项指标均低于对照组,差异具有统计学意义,P<0.05。结论在常规治疗的基础上采用有创-无创序贯通气治疗AOPP引发的IMS,恢复患者血清胆碱酯酶及提高患者各项生命体征,可有效改善患者预后。Objective To study the efficacy of invasive noninvasive sequential ventilation in the treatment of respiratory failure caused by intermediate syndrome(IMS)of acute organophosphorus pesticide poisoning(AOPP).Methods Three hundred and thirteen cases of IMS treated in our hospital from January 2019 to July 2021 were randomly divided into experimental group(149 cases)and control group(164 cases).The control group was treated with invasive ventilator,and the experimental group was treated with invasive and non-invasive sequential ventilator on the basis of the control group.The prognosis and complications of the two groups were compared.Results The total effective rate of the experimental group was significantly higher than that of the control group(P<0.05).There was significant difference in serum cholinesterase between the two groups after treatment(P<0.05).The difference of the recovery time of consciousness,spontaneous breathing,60%recovery time of cholinesterase activity,hospital stay,total computer time and invasive ventilation time of the two groups was statistically significant(P<0.05).The recovery time of the experimental group was shorter than that of the control group.There was significant difference in nosocomial infection,respiratory and related pneumonia and weaning failure rates between the two groups(P<0.05),and the experimental group was lower than the control group.Conclusion The invasive non-invasive sequential ventilation is used to treat IMS caused by AOPP on the basis of routine treatment,restore serum cholinesterase and improve various vital signs,which can effectively improve the prognosis of patients.
