髂腹下-髂腹股沟神经阻滞联合局部神经阻滞在老年腹股沟疝修补术中的应用价值

摘　　要：目的分析研究超声引导下髂腹下-髂腹股沟神经阻滞联合局部神经阻滞在老年腹股沟疝修补术中的临床效果和应用价值。方法选择我院收治的腹股沟疝患者100例,随机数字表法将患者分为对照组和观察组,均为50例,分别实施硬膜外麻醉及超声引导下髂腹下-髂腹股沟神经阻滞联合术中生殖股神经局部阻滞,比较不同麻醉方式的麻醉效果、并发症发生率、手术相关指标、术后不同时点VAS评分等。结果两种麻醉方式有效率对比无统计学意义(P>0.05);与对照组相比,并发症总发生率、麻醉操作时长、术后首次进食时间及术后离床活动时间等方面观察组则更优(P<0.05);在术后镇痛方面,观察组在术后6 h及术后12 h评分低于对照组(P<0.05)。结论超声引导下髂腹下-髂腹股沟神经阻滞联合术中生殖股神经局部阻滞应用于老年腹股沟疝修补术,安全有效、并发症少、术后镇痛好,有利于老年患者快速康复,值得临床应用推广。Objective To analyze the clinical effect and application value of ultrasound-guided inferior iliac inguinal nerve block combined with local nerve block in elderly inguinal hernia repair.Methods One hundred patients with inguinal hernia treated in our hospital were randomly divided into control group and observation group,with 50 cases in each group.Epidural anesthesia and ultrasound-guided inferior iliac inguinal nerve block combined with intraoperative genitofemoral nerve block were performed respectively.The anesthetic effects,complication rates,operation related indexes and VAS score at different time points after operation.Results The effective rate of the two anesthesia methods was high,and the comparison was not statistically significant(P>0.05).Compared with the control group,the observation group had more advantages in the total incidence of complications,the length of anesthesia operation,the time of first postoperative eating and the time of postoperative out of bed activities(P<0.05).In terms of postoperative analgesia,the scores of the observation group at 6 hours and 12 hours after operation were lower than those of the control group(P<0.05).Conclusion The application of ultrasound-guided infraabdominal iliac inguinal nerve block combined with intraoperative genitofemoral nerve block in elderly inguinal hernia repair is safe and effective,with less complications and good postoperative analgesia.It is conducive to the rapid recovery of elderly patients,and is worthy of clinical application and promotion.
