耻骨上正中切口双侧半月线入路Kugel修补术治疗老年人双侧腹股沟疝32例临床分析

摘　　要：目的采用耻骨上正中切口双侧半月线入路Kugel修补术治疗老年人双侧腹股沟疝,并分析其治疗效果。方法回顾性分析2008年3月至2020年12月我院收治老年人双侧腹股沟疝患者67例的临床资料,随机分为对照组35例,采用双侧Lichtenstein术治疗;观察组32例,采用耻骨上正中切口双侧半月线入路Kugel修补术治疗。将两组患者手术时间、术后住院时间、术后疼痛发生率及复发率进行比较。结果手术时间观察组(73.00±2.50)min明显短于对照组(92.00±3.00)min,有显著性差异(P<0.05);术后住院时间观察组(4.00±0.50)d与对照组(5.00±0.50)d相近,差异无统计学意义(P>0.05);术后疼痛发生率观察组1例(3.13%)低于对照组6例(17.14%),有显著性差异(P<0.05);所有病例均随访9~12个月,复发率观察组0例,低于对照组3例(8.57%),有显著性差异(P<0.05)。结论耻骨上正中切口双侧半月线入路Kugel修补术与Lichtenstein术均可作为老年人双侧腹股沟疝治疗的手术方法,但耻骨上正中切口双侧半月线入路Kugel修补术手术时间更短,术后疼痛发生率与复发率更低,值得推广应用。Objective To treat bilateral inguinal hernia in the elderly by Kugel repair through bilateral semilunar approach through suprapubic median incision,and to analyze its therapeutic effect.Methods The clinical data of 67 elderly patients with bilateral inguinal hernia treated in our hospital from March 2008 to December 2020 were retrospectively analyzed.They were randomly divided into 35 cases in the control group and treated with bilateral Lichtenstein operation;32 cases in the observation group were treated with Kugel repair through bilateral semilunar approach through suprapubic median incision.The operation time,postoperative hospital stay,postoperative pain incidence and recurrence rate were compared between the two groups.Results The operation time in the observation group(73.00±2.50)min was significantly shorter than that in the control group(92.00±3.00)min(P<0.05);The postoperative hospital stay in the observation group(4.00±0.50)d was similar to that in the control group(5.00±0.50)d,and there was no significant difference(P>0.05);The incidence of postoperative pain in 1 case(3.13%)in the observation group was significantly lower than that in 6 cases(17.14%)in the control group(P<0.05);All cases were followed up for 9-12 months.The recurrence rate of 0 cases in the observation group was lower than that of 3 cases(8.57%)in the control group(P<0.05).Conclusion Kugel repair through bilateral semilunar approach through suprapubic median incision and Lichtenstein can be used as surgical methods for the treatment of bilateral inguinal hernia in the elderly.However,Kugel repair through bilateral semilunar approach through suprapubic median incision has shorter operation time and lower postoperative pain incidence and recurrence rate,which is worthy of popularization and application.
