赤凤迎源针刺法对肩关节周围炎功能障碍及等速肌力的影响

摘　　要：目的观察赤凤迎源复式针刺手法对肩关节周围炎功能障碍及肩部肌群等速肌力的影响。方法采用随机数字表法将60例肩关节周围炎患者分为对照组(30例)和观察组(30例)。观察组予赤凤迎源复式针刺手法,对照组予平补平泻单式针刺手法。比较两组治疗前后疼痛视觉模拟量表(VAS)评分、肩关节活动度(ROM)和等速肌力变化。结果治疗前,两组VAS评分、肩关节ROM和肩部外展及前屈肌群等速肌力指标(PT、AP)比较,差异均无统计学意义(P>0.05)。治疗后,两组VAS评分、肩关节ROM和肩部外展及前屈肌群等速肌力指标(PT、AP)均较同组治疗前明显改善(P<0.05),且观察组均优于对照组(P<0.05)。结论赤凤迎源针刺法治疗肩关节周围炎可缓解疼痛,改善肩关节活动度,提高肩关节周围肌力,疗效优于平补平泻单式针刺。Objective To observe the effects of the complex acupuncture manipulation Chi Feng Ying Yuan(phoenix spreading its wings)on the dysfunction and shoulder isokinetic muscle strength in capsulitis of the shoulder.Method Sixty patients with capsulitis of the shoulder were divided into a control group(30 cases)and an observation group(30 cases)using the random number table method.The observation group was treated with the complex acupuncture manipulation Chi Feng Ying Yuan,while the control group was given the moderate reinforcing-reducing acupuncture manipulation.Before and after treatment,the visual analog scale(VAS)score,range of motion(ROM)of the shoulder joint,and isokinetic strength testing result were compared.Result Before treatment,there were no significant differences in the VAS score,ROM of the shoulder joint,and the isokinetic strength parameters[peak torque(PT)and average power(AP)]of the shoulder abductor and flexor muscles(P>0.05).After treatment,the VAS score,ROM of the shoulder joint,and isokinetic strength parameters(PT and AP)of the shoulder abductor and flexor muscles significantly improved in both groups(P<0.05),and were better in the observation group than those in the control group(P<0.05).Conclusion Chi Feng Ying Yuan acupuncture can release pain,improve the ROM of the shoulder,increase the strength of shoulder muscles in treating capsulitis of the shoulder,and produce more significant efficacy than the moderate reinforcing-reducing acupuncture manipulation.
