抗心磷脂抗体、抗β_(2)糖蛋白Ⅰ抗体、抗dsDNA抗体、抗C1q抗体在复发性流产诊断中的价值分析

摘　　要：目的探讨与抗磷脂综合征(APS)相关的抗心磷脂抗体(ACA)和抗β_(2)糖蛋白Ⅰ抗体(A-β_(2)GPⅠ)各3种分型抗体以及与系统性红斑狼疮(SLE)相关的抗dsDNA抗体和抗C1q抗体在复发性流产(RM)诊断中的价值分析。方法试验分为RM实验组和健康对照组两组研究对象,通过运用SPSS23.0软件统计分析比较两组研究对象的ACA、A-β_(2)GPⅠ、抗dsDNA抗体、抗C1q抗体的均值和阳性率;采用ROC曲线分析AUC数值,并探讨几种诊断指标的诊断价值;并采用人工方法通过计算公式分别统计计算各指标的敏感度、特异度、阳性预测值和阴性预测值。结果RM组的ACA-IgM、A-β_(2)GPⅠ-IgM、抗dsDNA抗体、抗C1q抗体的均值和阳性率与健康对照组相比具有显著性差异(P<0.01);ROC曲线结果分析显示,RM组A-β_(2)GPⅠ-IgM诊断效能较突出,AUC值可达0.897,其余3项检测指标诊断效能一般。结论ACA-IgM、A-β_(2)GPⅠ-IgM、抗dsDNA抗体和抗C1q抗体均与RM密切相关,对RM具有一定的诊断效能,可以作为诊断RM的血清学指标。Objective Objective to investigate the value of anticardiolipin antibody(ACA)and antiβ_(2)glycoprotein I antibody(A-β_(2)GP I)related to antiphospholipid syndrome(APS)and the value of anti dsDNA antibody and anti C1q antibody related to systemic lupus erythematosus(SLE)in the diagnosis of recurrent miscarriage(RM).Methods The experiment was divided into RM experimental group and healthy control group.The mean and positive rate of ACA,A-β_(2)GPⅠ,anti dsDNA antibody,anti-C1q antibody in the two groups were compared by using spss23.0software;using ROC curve to analyze the AUC value,and to explore the diagnostic value of several diagnostic indicators;and using artificial methods to calculate the sensitivity,specificity,positive predictive value and negative predictive value of each index through calculation formula.Results The mean and positive rates of ACA-IgM,A-β_(2)GPⅠ-IgM,anti dsDNA antibody and anti-C1q antibody in RM group were significantly different from those in healthy control group(P<0.01);the results of ROC curve showed that the diagnostic efficiency of A-β_(2)GPⅠ-IgM in RM group was higher than that in healthy control group,the AUC value was 0.897,and the diagnostic efficiency of the other three detection indexes was general.Conclusion ACA-IgM,A-β_(2)GPⅠ-IgM,anti dsDNA antibody and anti-C1q antibody are closely related to infertility.They have certain diagnostic efficacy for RM and can be used as serological indicators for the diagnosis of RM.
