摘 要:目的探讨纳米粒子(中空介孔二氧化硅纳米粒子)载雷帕霉素药物涂层球囊抗动脉粥样硬化模型抗兔腹主动脉狭窄的作用及机制。方法兔动脉粥样硬化模型,随机分为:假手术组、普通球囊扩张组、纳米粒子载雷帕霉素球囊扩张组(纳米雷帕霉素球囊组)各8只。沿导丝按超声指导长度送入球囊扩张腹主动脉,术后取耳缘血,监测相关炎性因子血清超敏c反应蛋白(hs-CRP)、超氧化物歧化酶(SOD)、纤维蛋白原浓度(FIB)。分别于术后4周、8周再次监测耳缘血监测相关炎性因子,及8周超声测量腹主动脉腹主动脉内径。8周后处死动物,取腹主动脉1cm,行HE染色光镜检查。结果hs-CRP、FIB,普通球囊组比假手术组增高,纳米雷帕霉素球囊组比普通球囊组降低,具有统计学差异(P<0.05)。SOD,纳米雷帕霉素球囊组与假手术组、普通球囊组比较明显升高,具有统计学差异(P<0.01)。术前、术后8周超声测量腹主动脉内径:纳米雷帕霉素药物涂层球囊组明显优于假手术组(P<0.01),纳米粒子载雷帕霉素药物涂层球囊组优于普通球囊组,普通球囊组优于假手术组(P<0.05)。HE染色光镜检查:假手术组管腔狭窄,血管内膜增厚、有连续较高隆起,有大量泡沫细胞形成,中膜平滑肌细胞明显增殖,排列紊乱。普通球囊组管腔扩张,内皮细胞脱落,内膜增厚,泡沫细胞、平滑肌细胞和纤维组织增生。纳米雷帕霉素球囊组管腔扩张,明显抑制内膜、平滑肌细胞、纤维组织增生。结论纳米雷帕霉素球囊组较普通球囊组明显抑制动脉粥样硬化模型兔腹主动脉狭窄,机制为纳米粒子的高载药性、高渗透性、缓释性,可以提高雷帕霉素抗动脉硬化炎症反应、抗氧化、降低纤维蛋白原浓度。Objective To investigate the effect and mechanism of the nanoparticles(hollow interporous silica nanoparticles)carrying rapamycin drug-coated balloon to treat the atherosclerosis model for rabbit abdominal aortic stenosis.Methods Rabbit atherosclerosis model was randomly divided into.false surgical group,common balloon dilation group and Nanoparticles(hollow interporous silica nanoparticles)carrying rapamycin balloon expansion group(hereinafter referred to as.nano repamycin balloon group),8 of each.The abdominal aorta was extended into the balloon along an ultrasound-guided length,and postoperative ear margin was taken to monitor the associated inflammatory factor serum hypersensitivity c reactive protein(hs-CRP),superoxide dismutase(SOD),fibrinogen concentration(FIB).The ear-edge blood monitoring-related inflammatory factors were monitored again at 4 weeks and 8 weeks after surgery.Internal diameter of abdominal aorta with 8-week ultrasound.Animal died suddenly after 8 weeks,Take the abdominal aorta for1 cm,HE staining,Optical examination.Results Hs-CRP,FIB,the common balloon group,higher than the pseudosurgical group,and the nanrepamycin balloon group lower than the common balloon group,had statistical differences in P<0.05.SOD,the nanrepamycin balloon group increased significantly compared with pseudosurgical and ordinary balloon groups,with statistical differences in P<0.01.Measurement of abdominal aortic diameter by preoperative and 8 weeks ultrasound:The nanoparticle-loaded rapamycin drug-coated capsule group was significantly better than the pseudosurgical group,P<0.01.The nanoparticle-loaded rapamycin drug-coated balloon group is superior to the common balloon group,The common balloon group was superior to the pseudosurgical group,P<0.05.HE staining,Optical examination:In the pseudosurgical group,the lumen was narrow,thickened vascular intima,continuous high uplift,a large number of foam cells formed,and medium membrane smooth muscle cells proliferated and arranged.Conclusion The lumen expansion of the co
