肢体固定出血检测装置联合改良斜坡侧卧位在减少股动脉穿刺术后并发症中的应用

摘　　要：目的探讨自制肢体固定带及出血检测装置配合改良斜坡侧卧位对减少股动脉穿刺术后并发症的应用效果。方法选取符合标准的120例经股动脉穿刺行介入手术的患者,随机分为观察组(n=60)和对照组(n=60),对照组术后采用传统护理方法,观察组用自制肢体固定带及出血检测装置配合改良斜坡侧卧位对患者进行术后护理,比较两组患者穿刺点出血发生率、尿潴留发生率、腰背部疼痛程度及术后血压变化。结果观察组术后出血发生率(6.67%)低于对照组(11.7%),术后尿潴留发生率(1.7%)低于对照组(5.0%),腰背部疼痛评分(4.30±2.78)低于对照组(7.30±2.61)(P<0.05),手术前后血压波动低于对照组(P<0.05)。结论股动脉穿刺术后压迫止血过程中应用自制肢体固定带及出血检测装置配合改良斜坡侧卧位,可以降低穿刺点出血发生率、尿潴留发生率,减轻腰背部疼痛,降低血压波动,改善患者的就医体验,在临床护理中值得推广应用。Objective To discuss the application of self-made limb fixation belt and bleeding detection device combined with modified slope lateral recumbent position in reducing complications after femoral artery puncture.Methods A total of 120 patients met the inclusion criteria,who received interventional surgery via femoral artery puncture,were enrolled in this study.The patients were randomly and equally divided into observation group(n=60)and control group(n=60).As postoperative nursing care,the traditional nursing method was adopted for the patients of the control group,while self-made limb fixation belt and bleeding detection device combined with modified slope lateral recumbent position was employed for the patients of the observation group.The incidence of bleeding at puncture point,the incidence of urinary retention,the degree of lumbodorsal pain,and postoperative changes in blood pressure were compared between the two groups.Results In the observation group,the incidence of postoperative bleeding at puncture point,the incidence of postoperative urinary retention and the lumbodorsal pain score were 6.67%,1.7%and(4.30±2.78)points respectively,which were obviously lower than 11.7%,5.0%and(7.30±2.61)points respectively in the control group(P<0.05).The postoperative fluctuation extent of blood pressure in the observation group was lower than that in the control group(P<0.05).Conclusion For the postoperative compression hemostasis of puncture point after femoral artery puncture,the use of self-made limb fixation belt and bleeding detection device together with modified slope lateral recumbent position can effectively reduce the incidences of postoperative bleeding at puncture point,urinary retention,the degree of lumbodorsal pain,and the fluctuation extent of blood pressure,which can remarkably improve the patients’experience of medical treatment.Therefore,this technique is worth popularizing in clinical nursing practice.
