摘 要:目的探讨乳腺癌化疗相关主观认知抱怨患者的大脑功能网络中心性的改变及其与认知功能的关系。方法纳入化疗后出现主观认知抱怨的乳腺癌患者40例(SCC组)和化疗前乳腺癌患者51例(BC组),进行静息态功能MRI扫描。基于图论分析,分别使用度中心性(DC)和特征向量中心性(EC)表征不同频段下(经典频段、slow-5频段、slow-4频段)全脑局部和全局功能连接度。使用两样本t检验比较两组被试脑功能网络中心性的差异,并进一步利用偏相关分析其与认知功能的关系。结果与BC组相比,SCC组在经典频段左侧枕中回及右侧楔叶/枕中回DC值更低,在slow-5频段右侧颞中回/角回DC值更高(GRF校正,体素水平P<0.001,簇水平P<0.05)。左侧枕中回(r=0.514,P=0.029)及右侧楔叶/枕中回(r=0.526,P=0.025)DC值与视空间及执行功能显著正相关,右侧颞中回/角回DC值与主观认知抱怨显著负相关(r=-0.505,P=0.033)。对于EC,两组在各频段均没有发现显著性差异的脑区。结论乳腺癌化疗后出现主观认知抱怨患者局部内在功能网络连接发生改变,并与较差的主观认知表现相关。Objective To explore the alterations of brain functional network centrality and its relationship with cognitive function of chemotherapy-related cognitive impairment in breast cancer. Methods Forty breast cancer patients with subjective cognitive complaints after chemotherapy(SCC group) and fifty-one breast cancer patients before chemotherapy(BC group) were recruited and underwent resting-stating fMRI scanning.Based on graph theory analysis, degree centrality(DC) and Eigenvector centrality(EC) were used to characterize the local and global functional connectivity degree of the whole brain network in different frequency bands(typical band: 0.01~0.08 Hz;slow-5 band: 0.01~0.027 Hz;slow-4 band: 0.027~0.073 Hz).Additionally, subjective and objective cognition test swere adopted to measure cognitive performance of all the subjects.Two-sample t-test was used to evaluate the difference of brain functional network centrality between the two groups, and furtherpartial correlation analysis was used to analyze the relationship between these network variables and cognitive function. Results Compared to BC group, in typical band, SCC group had significantly lower DC values in left middle occipital gyrus and right cuneus/middle occipital gyrus, while in slow-5 band had significant higher DC in right middle temporal gyrus/angular gyrus than BC group. The DC of left middle occipital gyrus(r=0.514,P=0.029) and right cuneus/middle occipital gyrus(r=0.526,P=0.025) were positively correlated with visuospatial and executive function, and the DC of right middle temporal gyrus/angular gyrus was negatively correlated with subjective cognitive complaints(r=-0.505,P=0.033).While there was no significant difference in brain regions between the two groups in each frequency band for EC. Conclusion The local intrinsic functional networks connectivity in breast cancer patients with subjective cognitive complaints after chemotherapy-related which was correlated with worse subjective cognitive performance.
