摘 要:目的研究精神分裂症共病代谢综合征(MS)患者肝、胰脂肪含量的变化,探讨其与相关临床指标的相关性。方法共纳入行腹部MR IDEAL-IQ扫描的精神分裂症患者100名,按照是否共病MS分组,观察组(合并MS)43人,对照组(不伴MS)57人,对其进行肝脏及胰腺的质子密度脂肪分数(PDFF)测量。采用非参数检验比较两组肝脏和胰腺PDFF值、体重、体质指数(BMI)、甘油三酯、空腹血糖、年龄及身高的差异;采用Spearman秩相关系数,分析肝、胰PDFF的相关性,肝、胰PDFF分别与年龄、BMI、空腹血糖、甘油三酯的相关性。结果观察组肝、胰PDFF值均高于对照组,差异具有统计学意义;观察组肝与胰的PDFF呈正相关(r=0.771,P=0.000),对照组也呈正相关(r=0.362,P=0.006);对照组体重、BMI、甘油三酯、空腹血糖均大于观察组,差异具有统计学意义;观察组肝脏和胰腺的PDFF值均与BMI、甘油三酯、空腹血糖呈正相关。结论精神分裂症共病MS时,肝、胰脂肪含量明显升高,且均与甘油三酯、空腹血糖和BMI呈正相关。肝脏与胰腺的脂肪含量呈正相关,当共病MS时,相关性更明显。Objective To explore the changes of liver and pancreatic fat content in patients with schizophrenia and metabolic syndrome,and to study the correlation between liver and pancreatic fat content and other related clinical indicators.Methods A total of 100 patients with schizophrenia who underwent abdominal MR Ideal-IQ scan were divided into groups according to whether or not they had comorbid MS.The PDFF values of liver and pancreas were measured in 43 patients in the observation group(combined with MS)and 57 patients in the control group(without MS).Non-parametric tests were used to compare the PDFF values of liver and pancreas fat-Weight,body mass index(BMI),triglyceride(TG),fasting blood glucose(FPG),Age and Height between the two groups.Spearman rank correlation coefficient was used for non-normal distribution data to analyze the correlation between liver and pancreatic fat content and whether there was correlation between liver and pancreatic fat content and age,BMI,FPG and TG,respectively.Results The PDFF values of liver and pancreas in the observation group were higher than those in the control group(15.74%、10.01%vs 4.90%、3.33%),the difference was statistically significant.The PDFF of liver and pancreas in the observation group was positively correlated(r=0.771,P=0.000),and that in the control group was also positively correlated(r=0.362,P=0.006).Weight,BMI,TG and FPG in the control group were all higher than those in the observation group,and the differences were statistically significant.PDFF values in liver and pancreas of observation group were positively correlated with BMI,TG and FPG.Conclusion The fat content in liver and pancreas was significantly increased in patients with schizophrenia comorbidity withMS,andthey were both positively correlated with triglyceride,fasting blood glucose and BMI.Liver and pancreas fat content was positively correlated in both groups,and the correlation was more significant in MS comorbidity of schizophrenia.
