余杭地区人乳头瘤病毒感染情况及风险因素调查

摘　　要：目的调查余杭地区人乳头瘤病毒(human papilloma virus,HPV)的感染情况,并分析HPV感染的风险因素。方法收集余杭地区2019年9月—11月进行HPV检测的334500名女性完整信息,整理其检测结果。结果334500名女性中,HPV阳性者3767例,占比1.13%;HPV阳性者中HPV52的阳性数(581例,占比15.42%)高于其他亚型,55岁以上女性的HPV阳性率高于其他年龄,差异均有统计学意义(P<0.05)。HPV阳性者与阴性者在孕次、饮酒上差异无统计学意义(P>0.05),在居住地、产次、流产史、性伴侣个数、首次性生活年龄、吸烟、避孕方式上差异有统计学意义(P<0.05)。将上述有差异资料带入Logistic回归方程计算,发现上述因素均是HPV感染的风险因素(P<0.05)。结论余杭地区HPV阳性检出率为1.13%,且风险因素较多,应重点关注55岁以上女性的筛查管理,加大疾病相关知识的宣教力度,从而有效减少HPV感染。Objective To study HPV(human papilloma virus)infection conditions and risk factors in Yuhang Area.Methods The complete information of 334500 women who underwent HPV testing in Yuhang Area from September to November in 2019 was obtained,and their testing results were sorted out.Results Among 334500 women,3767 were HPV positive,accounting for 1.13%.Among positive cases,HPV subtype distribution rate of HPV52 was 15.42%(581 cases),which was significantly higher than other subtypes;HPV positive rate in women aged over 55 was significantly higher than those of other age groups(P<0.05).The pregnancy frequency and drinking history between positive cases and negative cases showed no statistically significant difference(P>0.05);the residence place,parity,induced abortion history,sex mate,age of first sex,smoking history and contraception methods between HPV positive group and HPV negative group were found to be statistically significant(P<0.05).Logistic regression equation indicated the above-mentioned difference aspects were the risk factors of HPV infection(P<0.05).Conclusions In Yuhang Area,HPV infection rate was about 1.13%,and there were multiple risk factors of HPV infection.The screening and management for women aged over 55 deserved attention,and the propaganda of disease-related knowledge should be improved,so the HPV infection rate could be reduced.
