外用复方五凤草液对结核性溃疡患者MDM2、p53的影响

摘　　要：目的:通过观察复方五凤草液对结核性溃疡患者MDM2、p53表达的影响,探讨其治疗结核性溃疡可能的作用机制。方法:选取南京中医药大学附属南京市中西医结合医院通过病理学证实为结核性溃疡患者76例,随机分为实验组和对照组,每组各38例。两组均给予规范抗结核治疗,同时实验组予以复方五凤草液外用换药,对照组予以康复新液外用换药,治疗28 d。观察两组患者治疗后第3、7、14、28天的创面愈合率;两组患者创面腐去时间及上皮生长时间;于治疗前、治疗后第3、7、14、28天收集患者血浆,使用ELISA法检测血浆中MDM2、p53蛋白的表达水平,RT-qPCR技术检测血浆中MDM2、p53 mRNA表达水平;于治疗前、治疗后第28天收集患者创面组织,使用WesternBlot法检测其中MDM2、p53蛋白的表达水平。结果:与治疗后第7天比较,治疗后第14、28天时两组患者创面愈合率明显升高(P<0.05);与对照组比较,治疗后第28天时实验组的创面愈合率明显升高(P<0.05)。治疗前,实验组与对照组血浆中MDM2、p53蛋白及mRNA表达情况差异无统计学意义(P>0.05);与治疗前相比,第7、14、28天时,两组患者血浆中MDM2、p53蛋白及mRNA表达水平均有明显改善(P<0.05),而与对照组同一时间相比,实验组血浆中MDM2蛋白及mRNA表达水平升高更明显(P<0.05),p53蛋白及mRNA表达水平降低更明显(P<0.05)。与治疗前相比,两组患者治疗后创面组织中MDM2蛋白表达水平均升高,p53蛋白表达水平均降低;与对照组相比,实验组创面组织中MDM2、p53蛋白表达改善更明显。结论:复方五凤草液能够促进结核性溃疡愈合,其机制可能与上调MDM2表达、下调p53表达,从而调控巨噬细胞极化失衡有关。Objective:To observe the effect of Compound Wufengcao Solution on the expressions of MDM2 and p53 in patients with tuberculous ulcer,and to explore the possible mechanism of Compound Wufengcao Solution in treating tuberculous ulcer.Methods:76 patients with tuberculous ulcer were randomly divided into the study group and the control group,with 38 cases in each group.Both groups received standardized anti-tuberculosis treatment;on which basis,the study group was treated with external application of Compound Wufengcao Solution,whereas the control group was treated with external application of Kangfuxin Solution.The treatment duration was 28 days.The wound healing rate was observed on 3rd,7th,14th and 28th days in the two groups,and the wound decay time and the epithelial growth time were also observed in the two groups.Plasma expressions of MDM2 and p53 were detected by ELISA,and the mRNA expressions of MDM2 and p53 were detected by RT-qPCR on 3rd,7th,14th and 28th days.The protein expressions of MDM2 and p53 in the wound tissues were observed by Western blot before and after the treatment in the two groups.Results:The wound healing rate was higher on 7th day compared to that on 14th and 28th days in the two groups(P<0.05);the wound healing rate was significantly higher in the study group than that in the control group on 28th day(P<0.05).There were no statistical differences in protein and mRNA expressions of MDM2 and p53 between the two groups before the treatment(P>0.05);which were significantly improved in the two groups on 7th,14th and 28th days(P<0.05);compared to those in the control group,the protein and mRNA expressions of MDM2 were significantly higher(P<0.05),and the protein and mRNA expressions of p53 were significantly lower in the study group at the same time point(P<0.05).The protein expression of MDM2 in wounded tissues was higher,and the protein expression of p53 in wounded tissues was lower after the treatment compared to those before the treatment in the two groups;of which the improvements in the
