基于“土爰稼穑”探讨健脾方药修复干细胞“土壤”细胞外基质紊乱防治胃癌变的科学内涵

摘　　要：细胞外基质(ECM)是干细胞赖以生存的"土壤",对干细胞有容纳、支撑、营养及保护作用,决定干细胞"种子"的命运。基于中医理论和临床实践,结合现代医学、分子生物学文献,本文提出胃癌的发生可能是ECM微环境(土壤)紊乱导致干细胞(种子)发生的异常迁移、分化、增殖现象,与中医"脾土虚损,稼穑失调"病机相契合,而健脾方可能通过改善ECM微环境调控干细胞活性,遏制胃黏膜恶变,即干预"ECM紊乱-干细胞失调-胃癌变"演进过程而阻遏其进展。可进一步诠释中医"健脾益气、扶正抗邪"防治胃黏膜恶性转变的科学内涵。Extracellular matrix(ECM),which serves as the“soil”for stem cells,can accommodate,support,nourish and protect stem cells,and thus determines the fate of stem cells.These characteristics of ECM are similar to the TCM theory of“Tu Yuan Jia Se”.Based on the TCM theory,clinical practice as well as review of medical and molecular biological literature,it is found that etiological and pathogenetic hypothesis may be ECM microenvironment(soil)disorder causing abnormal migration,differentiation,and proliferation of stem cells(seeds),which resembles“deficiency of the spleen soil leading to disorder of Jia Se”.Additionally,invigorating-spleen prescriptions may improve ECM microenvironment to regulate the activity of stem cells and suppress malignant transformation of the gastric mucosa,that is,to intervene in the evolution process of“ECM disorder-stem cell disorder-gastric cancer”to stop its progress.This article aimed to provide scientific connotation of TCM“invigorating the spleen and strengthening the body to resist pathogen”in prevention and treatment of gastric precancerous lesions.
