摘 要:目的探讨1.5 T磁共振成像(MRI)T_(2)WI-SPAIR序列三点计划扫描技术(3PPS)在腋神经损伤诊断中的应用价值。方法59例经神经肌电图检查确诊(金标准)腋神经损伤患者,进行常规冠状T_(2)WI-SPAIR序列和3PPS技术T_(2)WI-SPAIR序列扫描,对比分析两组图像腋神经的图像特征及灵敏度、特异性评估。结果常规MRI检查确诊40例,诊断符合率为68%;1.5 T MRI三点计划扫描技术确诊56例,诊断符合率为95%;诊断灵敏度和特异度三点计划扫描技术均高于常规MRI,差异有统计学意义(P<0.05)。56例患者T_(2)WI-SPAIR序列3PPS技术扫描影像均能同一层面直观、清楚地显示腋神经走形及损伤程度。结论1.5 T MRI三点计划扫描技术具有较高的诊断符合率,且评估灵敏度、特异度较高,为临床诊治腋神经病变提供直观的影像学依据,值得推广应用。Objective To explore the diagnostic value of 3PPS technique using T_(2)WI-SPAIR sequence in axillary nerve injury on 1.5 T MRI.Methods Fifty-nine patients with brachial plexus injury diagnosis by electromyography(EMG)detection received 3PPS technique and conventional coronal T_(2)WI-SPAIR sequence at 1.5 T MRI were analyzed.Compare the image features,sensitivity and specificity of the two methods.Results Diagnostic coincidence rate was 68%(P>0.05)by conventional T_(2)WI-SPAIR sequence and 95%(P<0.05)by 3PPS technique.The sensitivity and specificity were higher by 3PPS technique than conventional T_(2)WI-SPAIR sequence.For 56 patients,3PPS technique could display the axillary nerve injury in the same image.Conclusion 3PPS Technique on 1.5 T MRI has higher diagnostic coincidence rate,sensitivity and specificity in displaying axillary nerve injury than conventional T2WI-SPAIR sequence.
