等级医院评审前后输血病历质量的调查分析

摘　　要：目的研究等级医院评审对临床输血工作规范化管理和持续改进的应用价值。方法随机抽取某院等级医院评审前(2017年1月—2018年6月)输血病历720份和评审后(2019年1月—2020年6月)输血病历720份,比较评审前后输血病历书写质量,记录缺陷内容并进行分析。结果评审后,输血病历缺陷项除“血浆适应证不符”外,其他缺陷项发生率均低于评审前,差异均有统计学意义(P<0.05);零缺陷输血病历比例由48.33%上升至81.53%,中度缺陷和重度缺陷输血病历比例分别由18.61%、22.92%下降至2.36%、7.64%,差异均有统计学意义(P<0.05);评审前后的轻度缺陷病历比较,差异无统计学意义(P>0.05)。结论将等级医院评审工作制度化、常态化可促进医院临床输血管理,落实三级质控体系能显著提高输血病历质量。Objective To discuss the application value of grade hospital accreditation in the standardized and continuous improvement of Blood transfusion management.Methods A total of 720 blood transfusion medical records of a hospital-level hospital before the review(from January 2017 to June 2018)and 720 blood transfusion medical records after the review(from January 2019 to June 2020)were randomly examined,and compared their writing quality,then recorded and analyzed the defect contents.Results After the review,the incidence of all defect items in the blood transfusion medical record was lower than that before the review,except for"plasma indication inconsistency",and the difference was statistically significant(P<0.05).The proportion of zero-defective blood transfusion records was increased from 48.33%to 81.53%,and the proportion of mid-defective medical records and severe-defective medical records were decreased from 18.61%22.92%to 2.36%7.64%,the differences were statistically significant(P<0.05).There was no statistical significance in the comparison of mild-defective medical records before and after the accreditation(P>0.05).Conclusion Institutionalizing and normalizing the grade hospitals accreditation accreditation can promote the management level of clinical blood transfusion,and implementing the three-hierarchical quality control system can significantly improve the quality of blood transfusion medical records.
