掌侧入路锁定钢板内固定对不稳定桡骨远端骨折的疗效

摘　　要：目的分析掌侧入路下经锁定钢板开展内固定对不稳定桡骨远端骨折的疗效。方法将2017年1月—2019年12月本院收治的不稳定桡骨远端骨折患者80例作为研究对象,依据随机数表法分成对照组(于背侧入路下经锁定钢板开展内固定)和观察组(于掌侧入路下经锁定钢板开展内固定)各40例,比较两组的关节恢复优良率、手术用时、术后活动时间、愈合时间、尺偏角与掌倾角和并发症情况。结果观察组关节恢复优良例数占比和对照组比较差异无统计学意义(P>0.05)。两组手术时间对比差异无统计学意义(P>0.05)。观察组术后活动及愈合时间均早于对照组(P<0.05)。术后6个月,两组尺偏角与掌倾角均升高,但组间对比无显著差异(P>0.05)。观察组并发症出现少于对照组(P<0.05)。结论掌侧入路下经锁定钢板开展内固定对不稳定桡骨远端骨折疗效确切,能改善患者的腕关节功能,促进其术后恢复,减少并发症出现,值得采用。Objective To study the therapeutic effects of unstable distal radius fracture patients using the locking plate and internal fixation via palmar approach.Methods 80 unstable distal radius fracture patients undergoing the locking plate and internal fixation from January 2017 to December 2019 in our hospital were chosen as research subjects.According to random number table,patients were divided into control group(via dorsal approach)and observation group(via palmar approach),40 cases in each group.The good joint function recovery rate,operation time,postoperative ambulation time,healing time,radial inclination angle,palmar tilt and complications were compared.Results The proportions of good joint function recovery between groups were not significantly different(P>0.05);the operation time between groups was not significantly different(P>0.05);the postoperative ambulation time and healing time in the observation group were earlier than control group(P<0.05);after 6 months of surgery,the radial inclination angle and palmar tilt for two groups were increased,but no significant differences between the two groups were observed(P>0.05);the postoperative complications in the observation group were fewer than that in control group(P<0.05).Conclusion The locking plate and internal fixation via palmar approach can improve the patients’s wrist joint function,promote the fast recovery and reduce the postoperative complications.It is worthy of application.
