芪杞益肾胶囊治疗脾肾气虚兼血瘀型特发性膜性肾病临床观察

摘　　要：目的:观察芪杞益肾胶囊治疗特发性膜性肾病脾肾气虚兼血瘀证的临床疗效。方法:将符合纳入标准的80例患者随机分为两组各40例。对照组予西医基础治疗加氯沙坦钾片口服;治疗组在对照组基础上联合芪杞益肾胶囊口服;疗程均为16周。检测两组治疗前后24小时尿蛋白定量、血浆白蛋白、血清肌酐(Cr)、尿素氮(BUN)、谷草转氨酶(AST)、谷丙转氨酶(ALT)指标,比较两组临床疗效和中医证候疗效。结果:治疗后两组24h尿蛋白定量均较治疗前明显减少,血浆白蛋白较治疗前明显升高,治疗组24 h尿蛋白定量均明显低于对照组,血浆白蛋白水平均明显高于对照组,差异均有统计学意义(P﹤0.05);治疗组临床总有效率与中医证候疗效总有效率均高于对照组,差异均有统计学意义(P﹤0.05)。两组患者的肝、肾功能指标无异常。结论:芪杞益肾胶囊联合氯沙坦钾片口服等治疗特发性膜性肾病可降低患者24 h蛋白尿,提高血浆白蛋白水平,改善肾功能。Objective:To observe the clinical efficacy of Qiqi Yishen capsule on idiopathic membranous nephropathy with spleen-kidney-qi deficiency and blood stasis syndrome.Methods:80 cases of patients who met the inclusion criteria were randomly divided into two groups,40 cases in each group.The control group was treated with basic western medicine and oral losartan potassium tablets,while the treatment group was treated with oral Qiqi Yishen capsule on the basis of control group.For both groups,the course of treatment was 16 weeks.The indexes of 24hrUprV,plasma albumin,serum creatinine(Cr),blood urea nitrogen(BUN),aspartate aminotransferase(AST)and alanine aminotransferase(ALT)were detected in two groups before and after treatment.The clinical efficacy and TCM syndrome efficacy were compared between two groups.Results:After treatment in both groups,24hrUprV was significantly lower than before treatment,and plasma albumin was significantly higher than before treatment;after treatment,24hrUprV of treatment group was significantly lower than that of control group,and plasma albumin level of treatment group was averagely significantly higher than that of control group,and the difference was statistically significant(P﹤0.05).The clinical total effective rate and the total effective rate of TCM syndrome efficacy in treatment group were higher than in control group,and the difference was statistically significant(P﹤0.05).The indexes of liver and kidney function of patients in two groups were not abnormal.Conclusion:Qiqi Yishen capsule and oral losartan potassium tablets for treatment of idiopathic membranous nephropathy can reduce 24-hour proteinuria of patients,increase plasma albumin level of patients and improve renal function of patients.
