针灸治疗原发性痛经的选穴规律分析

摘　　要：目的系统评价针灸治疗原发性痛经的选穴规律。方法通过检索中国知网(CNKI)、维普数据库(VIP)、万方数据库和PubMed数据库,纳入建库至2021年3月公开发表的针灸治疗原发性痛经临床文献,统计并整理其针灸处方,运用数据挖掘技术分析其选穴用经规律。结果共纳入临床文献253篇,涉及68个穴位,总频数为1720。频数前10位的穴位依次是三阴交(217次)、关元(207次)、足三里(117次)、气海(113次)、地机(110次)、太冲(102次)、中极(98次)、次髎(85次)、血海(71次)、合谷(57次)。特定穴共50个,总频数为1202,非特定穴共18个,总频数为518。针灸治疗原发性痛经的选穴归经主要集中在任脉、脾经、膀胱经、胃经、肝经。聚类分析结果显示,针灸治疗原发性痛经的主要选穴可以分为4类:关元、三阴交属第1类;足三里、地机、气海、中极、太冲属第2类;血海、次髎、子宫、合谷、肾俞属第3类;肝俞、阴陵泉、脾俞、水道、命门、神阙、归来属第4类。结论针灸治疗原发性痛经有"远近配穴""前后配穴""阴阳同调"等特点。通过对主穴进行关联分析,从而为临床提供最新的治疗组方。Objective To systematically evaluate the analysis of acupuncture and moxibustion treatment of primary dysmenorrhea.Methods Through searching the China Knowledge Network(CNKI),VIP database(VIP),Wanfang database and PubMed database,the clinical literature on acupuncture and moxibustion treatment of primary dysmenorrhea published from the establishment of the database to March 2021 was included,and the acupuncture prescriptions were counted and sorted.Data mining technology was used to analyze the law of selecting acupoints and using meridians.Results A total of 253 clinical literatures were included,involving 68 acupoints,with a total frequency of 1720.The top 10 acupuncture points in frequency are Sanyinjiao(217 times),Guanyuan(207 times),Zusanli(117 times),Qihai(113 times),Diji(110 times),Taichong(102 times),Zhongji(98 times),Ciliao(85 times),Xuehai(71 times),Hegu(57 times).There are a total of 50 specific acupoints with a total frequency of 1202,and 18 non-specific acupoints with a total frequency of518.Acupuncture and moxibustion for the treatment of primary dysmenorrhea mainly focus on Ren channel,the pleen meridian,bladder meridian,stomach meridian and liver meridian.The cluster analysis results showed that the main selection of acupuncture points for the treatment of primary dysmenorrhea can be divided into four categories:Guanyuan and Sanyinjiao belong to the first category;Xuehai,Ciliao,Zigong,Hegu and Shenshu belong to the third category;Ganshu,Yinlingquan,Pishu,Shuidao,Mingmen,Shenque,and Guilai belong to the fourth category.Conclusion Acupuncture and moxibustion treatment of primary dysmenorrhea has the characteristics of"distal-proximal points combination","anterior-posterior point association",and"the mutual regulation of yin and yang".Through the correlation analysis of the main acupoints,the latest treatment prescriptions can be provided for the clinic.
