千山活血膏治疗95例踝关节创伤性关节炎的临床疗效

摘　　要：目的观察千山活血膏外敷治疗踝关节创伤性关节炎的临床疗效。方法选取2020年4月—2020年12月期间北京市昌平区中西医结合医院颈肩腰腿痛门诊收治的踝关节创伤性关节炎患者95例,所有患者均外敷千山活血膏治疗。治疗4周后,观察比较95例踝关节创伤性关节炎患者治疗前后视觉模拟量表(Visual analogue scale,VAS)评分、关节活动范围(Range of motion,ROM)评分,分析治疗后临床疗效、不良反应发生情况和复发率。结果95例患者治疗后VAS评分较治疗前明显降低,ROM评分较治疗前明显升高,差异有统计学意义(P<0.01)。治疗后95例踝关节创伤性关节炎患者显效53例(55.79%),有效40例(42.11%),无效2例(2.11%),总有效率为97.89%。治疗期间,95例踝关节创伤性关节炎患者中2例(2.11%)出现关节疼痛,其余均未出现关节肿胀等其他不良反应发生;随访1个月后4例(4.21%)复发。结论千山活血膏外敷法治疗踝关节创伤性关节炎临床效果较佳,患者疼痛小、不良反应少,关节活动良好,是较为安全有效的治疗方案。Objective To observe and analyze the clinical efficacyof Qianshan Huoxue Plaster in the treatment of traumatic ankle arthritis.Method A total of 95 outpatients with traumatic ankle arthritis were continuously recruited from April 2020 to December 2020 in the Department of Neck-Shoulder-Waist-Leg Pain,Beijing Changping Hospital of Integrated Chinese and Western Medicine.The clinical data were collected and analyzed.All patients were treated with external application of Qianshan Huoxue Plaster for 4 weeks.The joint pain visual analogue scale(VAS)score and joint range of motion(ROM)before and after treatment were compared in patients with traumatic ankle arthritis.The therapeutic effective rate after treatment,the adverse reaction and the recurrence rate during 4-week follow-up were analyzed.Result After treatment,the VAS score of 95 patients was significantly lower than that before treatment,and the ROM score was significantly higher than that before treatment.The difference was statistically significant(P<0.01).After treatment,95 cases of traumatic arthritis of the ankle joint were markedly effective in 53 cases(55.79%),effective in 40 cases(42.11%),and ineffective in 2 cases(2.11%).The total effective rate was 97.89%.During the treatment period,2 of the 95 patients with traumatic ankle arthritis(2.11%)had joint pain,and the rest had no other adverse reactions such as joint swelling.During4-week follow-up,4 cases had recurrence,with a recurrence rate of 4.21%and no other adverse reactions were observed.Conclusion The external application of Qianshan Huoxue Plaster has a good effect in patients with posttraumatic ankle arthritis,who showed less pain,fewer complications,and good joint mobility.Qianshan Huoxue Plaster is a relatively safe and effective treatment plan,which is worthy of clinical application.
