摘 要:目的探讨反复性自然流产(RSA)患者的生殖道解脲支原体(MU)感染情况与血管相关因子、Th1/Th2分化的内在联系。方法回顾性选取2017年6月至2021年1月在湘西土家族苗族自治州人民医院就诊的RSA患者149例,根据是否合并生殖道MU感染将其分为MU(+)组63例、MU(-)组86例。采用放射免疫法检测两组患者血清及绒毛组织中血管相关因子[血管内皮生长因子(VEGF)、血管生成素-1(Ang-1)、血管生成素-2(Ang-2)、血管抑素(AS)、内皮抑素(ES)、基质金属蛋白酶组织抑制剂(TIMP)、可溶性血管内皮生长因子受体-1(s Flt-1)]的含量,采用酶联免疫吸附试验检测Th1/Th2相关细胞因子的含量。结果MU(+)组患者血清及绒毛组织中促血管生成因子VEGF、Ang-1、Ang-2的含量低于MU(-)组患者,差异均有统计学意义(P<0.05);MU(+)组患者血清及绒毛组织中血管生成抑制因子AS、ES、TIMP、s Flt-1的含量高于MU(-)组患者,差异均有统计学意义(P<0.05);MU(+)组患者血清及绒毛组织中Th1相关细胞因子IL-2、IFN-γ、TNF-α的含量高于MU(-)组患者,Th2相关细胞因子IL-4、IL-6、IL-10的含量低于MU(-)组患者,差异均有统计学意义(P<0.05)。结论生殖道MU感染可加重RSA患者的血管新生障碍以及Th1/Th2免疫平衡紊乱,可能促使RSA发生的重要原因之一。Objective To investigate the relationship between Ureaplasma urealyticum(MU)infection and vascular factors,Th1/Th2 differentiation in patients with recurrent spontaneous abortion(RSA).Methods A tatol of 149 cases of RSA patients in Xiangxi Tujia and Miao Autonomous Prefecture People’s Hospital during June 2017 to January 2021 were retrospectively selected as research subject,according to MU infection all patients were divided intoMU(+)group(n=63)and MU(-)group(n=86).The contents of vascular related factors[vascular endothelial growth factor(VEGF),angiopoietin-1(Ang-1),angiopoietin-2(Ang-2),angiostatin(AS),endostatin(ES),tissue inhibitor of matrix metalloproteinases(TIMP),soluble fms-like tyrosine kinase receptor-1(s Flt-1)]was detected by radioimmunoassay,the contents of Th1/Th2 related cytokines in serum and villus tissue was detected by enzyme-linked immunosorbent assay.Results Contents of angiogenic factors VEGF,Ang-1 and Ang-2 in serum and villus tissue of MU(+)group were lower than those of MU(-)group,the differences were statistically significant(P<0.05).Contents of angiogenesis inhibitors AS,ES,TIMP,s Flt-1 in serum and villus tissue of MU(+)group were were higher than those of MU(-)group,the differences were statistically significant(P<0.05).Contents of Th1-related cytokines IL-2,IFN-γand TNF-αin serum and villus tissue of MU(+)group were higher than those of MU(-)group,Th2-related cytokines IL-4、IL-6、IL-10 were lower than those of MU(-)group,the differences were statistically significant(P<0.05).Conclusion Reproductive tract MU infection can aggravate angiogenesis and Th1/Th2 immune balance disorder in RSA patients,which may be one of the important reasons for RSA.
