不同抗Ro52抗体亚型与SLE/SS/IIM患者临床表现的相关性研究

摘　　要：目的:探讨不同抗Ro52抗体与SLE/SS/IIM患者临床表现的相关性。方法:收集诊断明确且免疫学检测抗Ro52抗体阳性SLE/SS/IIM的患者血清及病例资料分别50/30/30份,阴性患者的病例资料分别SLE50/IIM30份;重组Ro52蛋白作为抗原,检测SLE/SS/IIM患者血清中抗Ro52抗体亚型(IgA、IgG、IgM);用t检验和卡方检验对数据进行分析。结果:①110份血清样本中,抗Ro52抗体亚型IgG的OD值水平(1.772±0.01287)明显高于IgM(1.592±0.02076,P<0.0001)和IgA(1.469±0.02816,P<0.0001)水平;IgM的水平也明显高于IgA的水平(P<0.0001);②在50份SLE患者中,抗Ro52抗体阳性关节炎的发生率高于阴性患者(P=0.0018),且IgG型患者关节炎的发生率低于非IgG型(P=0.0451);③在30份IIM患者中,抗Ro52抗体阳性患者ILD的发生率高于阴性患者(P=0.0221),且IgG型患者ILD的发生率更高(P=0.0261)。结论:SLE/SS/IIM患者血清中抗Ro52抗体的亚型主要是以IgG为主,其次为IgM;在SLE患者中,抗Ro52抗体阳性的患者关节受累的概率较大,抗Ro52抗体IgG亚型为主的患者的关节受累概率较小;在IIM患者中,抗Ro52抗体阳性的患者ILD发生率较高,且抗Ro52抗体IgG亚型为主的患者ILD的发生率较高。Objective:To investigate the correlation between different anti-Ro52 antibodies and clinical manifestations of SLE/SS/IIM patients.Methods:Randomly collected 50/30/30 serum and case datas of clear diagnosis SLE/SS/IIM patients with immunological detection of anti-Ro52 antibody-positive and of cases datas SLE/SS/IIM patients with anti-Ro52 antibody-negative,respectively;recombinant Ro52 protein was used as antigen,to detect anti-Ro52 antibody subtypes(IgA,IgG,IgM)in serum of SLE/SS/IIM patients;t-test andχ2 test will be used to analyse the statistical data.Results:①The OD value's level of anti-Ro52 antibody subtype IgG(1.772±0.01287)was significantly higher than that of IgM(1.592±0.02076,P<0.0001)and IgA(1.469±0.02816,P<0.0001)in 110 serum samples.The level of IgM was also significantly higher than that of IgA(P<0.0001);②In 50 patients with SLE,the incidence of anti-Ro52 antibody-positive arthritis was higher than that of negative patients(P=0.0018),and the incidence of arthritis in IgG patients was lower than that of non-IgG(P=0.0451);③In 30 IIM patients,the incidence of ILD was higher in anti-Ro52 antibody-positive patients than that of negative patients(P=0.0221),and the incidence of ILD was higher in the IgG patients(P=0.0261).Conclusion:The anti-Ro52 antibody subtypes in SLE/SS/IIM patients are mainly IgG,followed by IgM.In SLE patients,anti-Ro52 antibody-positive patients have a higher probability of joint involvement;the probability of joint involvement of patients with anti-Ro52 antibody IgG subtype is small;in patients with IIM,the incidence of ILD is higher in patients with positive the antibody,and the incidence of ILD is higher in patients with IgG subtype.
