伊布替尼致3例特殊不良反应的病例分析及文献复习

摘　　要：本文报道我院3例B细胞慢性淋巴增殖性肿瘤患者常规剂量应用伊布替尼后引起出血、房颤和乙肝病毒再激活的特殊不良反应。通过文献检索和综述,对这些患者的临床特征、基础疾病、药物治疗过程等进行临床分析。结果显示,伊布替尼与出血事件、房颤、乙肝病毒再激活事件的发生存在相关性,在使用伊布替尼的过程中,应严密监测患者的出凝血功能、乙肝病毒拷贝数和心脏电生理变化,及时发现和处理这些特殊不良事件是患者用药安全和持续治疗的重要保障。Three cases of special adverse reactions in patients with B-cell chronic lymphoproliferative disorders who received ibrutinib in our hospital were reported,including bleeding,atrial fibrillation and reactivation of hepatitis B virus.The clinical characteristics,underlying diseases and drug treatment process of the patients were comprehensively analyzed by literature search and review.The result showed that ibrutinib was associated with bleeding events,atrial fibrillation and reactivation of hepatitis B virus.Coagulation function,HBV copy level and cardiac electrophysiological changes should be closely monitored during the use of ibrutinib.Timely detection and treatment of these adverse events is an important guarantee for medication safety and continuous treatment of patients.
