摘　　要：目的:基于中国医院药物警戒系统(CHPS)及时监测慢性心力衰竭合并急性肝功能衰竭患者,探讨临床药师为危重患者提供药学监护的作用。方法:通过CHPS监测到慢性心力衰竭合并急性肝功能衰竭患者,当患者出现心力衰竭、低钠、肺部感染、肝损伤时,密切监测相关指标,根据患者症状、尿量、血压等及时提供药学监护。结果:临床药师参与慢性心力衰竭合并急性肝功能衰竭等多种疾病患者的药物治疗过程,评估患者情况,根据证据级别高的文献结果,调整药物,患者病情得到改善。结论:临床药师利用CHPS及时发现慢性心力衰竭合并急性肝功能衰竭患者,主动进行药学监护,促进合理用药,提高医疗质量。Objective:Based on Chinese hospital pharmacovigilance system(CHPS)to timely monitor patients with chronic heart failure complicated with acute liver failure,and explore the role of clinical pharmacists in providing pharmaceutical care for critically ill patients.Methods:Patients with chronic heart failure complicated with acute liver failure were monitored through CHPS.When the patient developed heart failure,hyponatremia,lung infection and liver injury,clinical pharmacists closely monitor the relevant indicators and provide pharmaceutical care in time according to the patient s symptoms,urine volume and blood pressure.Results:Clinical pharmacists participated in the process of drug therapy for patients with chronic heart failure complicated with acute liver failure and other diseases,evaluate the situation of patients,and adjust the drug according to the literature results with high evidence level.The patient s condition improved.Conclusion:Clinical pharmacists use CHPS to timely find patients with chronic heart failure complicated with acute liver failure,and take the initiative to carry out pharmaceutical care to promote rational drug use and improve medical quality.
