左归四妙饮联合外阴熏洗治疗外阴营养不良肾虚湿热证临床研究

摘　　要：目的:探讨左归四妙饮联合外阴熏洗治疗外阴营养不良肾虚湿热证疗效以及其对患者免疫功能影响。方法:选取2019年2月—2020年9月就诊于该院妇科外阴营养不良肾虚湿热证患者60例,通过随机数字表非盲法分为2组,每组30例。试验组予以自拟左归四妙饮加减方口服,联合原方药渣煎汤外用熏洗。对照组予以复方丙酸氯倍他索软膏局部外涂。观察2组临床疗效、中医证候积分、细胞免疫功能以及不良反应变化情况。结果:①试验组总有效率是93.33%与对照组73.33%相比明显更高。②2组治疗后中医证候积分均低于治疗前,且试验组相比于对照组更优。③2组治疗后CD4;细胞百分比以及CD4;/CD8;比值均下降,且试验组比对照组下降更明显,CD8;细胞百分比较前有所提升,且试验组比对照组升高更明显。④2组治疗过程中均未发生严重不良反应,安全性较好。结论:左归四妙饮联合外阴熏洗治疗外阴营养不良肾虚湿热证临床效果较好,能够明显提高临床疗效,改善中医症状,有效改善患者细胞免疫功能,调节细胞免疫平衡,并且安全性高,值得在临床上应用。Objective:To explore the effect of Zuogui Si Miao Decoction combined with vulvar fumigation in the treatment of vulvar malnutrition with kidney deficiency and damp-heat syndrome and its influence on the immune function of patients.Methods:From February 2019 to September 2020,60 patients with vulvar dystrophy and kidney deficiency and damp-heat syndrome were selected and divided into 2 groups with 30 cases in each group by random number table unblinded.The experimental group was given the self-made Zuogui Si Miao Yin plus or minus prescription orally,combined with the original prescription medicine dregs decoction for external use,fumigation and washing.The control group was given local topical application of compound clobetasol propionate ointment.Observe the clinical efficacy,TCM syndrome score,cellular immune function and changes in adverse reactions in the 2 groups.Results:①The total effective rate of the experimental group was 93.33%,which was significantly higher than that of the control group 73.33%.② The scores of TCM syndromes in both groups after treatment were lower than those before treatment,and the test group was better than the control group.③ The percentage of CD4+ cells and the ratio of CD4+/CD8+ in the two groups decreased after treatment,and the decrease in the test group was more significant than that in the control group,the percentage of CD8+ cells increased than before,and the increase in the test group was more significant than the control group.④ No serious adverse reactions occurred during the treatment of the two groups,and the safety was good.Conclusion:Zuogui Si Miao Decoction combined with vulvar fumigation and washing has a good clinical effect in the treatment of vulvar dystrophy with kidney deficiency and damp-heat syndrome.It can significantly improve the clinical efficacy,improve TCM symptoms,effectively improve the patient’s cellular immune function,regulate cellular immune balance,and has high safety.It is worthy of clinical application.
