加减益气养阴化瘀方对早中期糖尿病肾病气阴两虚夹瘀证的疗效观察

摘　　要：目的:观察加减益气养阴化瘀方对早中期糖尿病肾病(DN)气阴两虚夹瘀证的临床疗效。方法:选取2019年8月—2020年12月该院收治的86例早中期DN气阴两虚夹瘀证患者为研究对象,随机分为对照组(n=43)和观察组(n=43)。对照组予以常规西医综合治疗,观察组在对照组治疗基础上予以加减益气养阴化瘀方治疗。2周为一个疗程,治疗4个疗程后观察临床疗效、中医证候积分、血糖相关指标、C反应蛋白(CRP)、同型半胱氨酸(Hcy)、肾功能相关指标水平。结果:观察组临床总有效率为81.40%,明显高于对照组的60.47%(P<0.05)。观察组治疗后2周、4周、8周的神疲乏力、腰膝酸软、手足心热和口渴欲饮症状评分均低于对照组(P<0.05)。两组治疗后空腹血糖(FBG)、餐后2h血糖和糖化血红蛋白(HbA1c)水平比较,差异无显著性(P>0.05)。观察组治疗后CRP、Hcy水平明显低于对照组(P<0.05)。观察组治疗后血肌酐(Scr)、尿素氮(BUN)、尿微量白蛋白排泄率(UAER)水平明显低于对照组(P<0.05)。结论:加减益气养阴化瘀方治疗早中期DN气阴两虚夹瘀证效果良好,可有效改善患者中医证候积分和肾功能,改善机体微炎症状态。Objective:To observe the clinical effect of modified Qi-Yin Huayu Recipe on early and mid-stage diabetic nephropathy (DN) with Qi-yin deficiency and blood stasis syndrome.Methods:From August 2019 to December 2020,86 patients with early and mid-term DN with Qi-yin deficiency and blood stasis syndrome were selected as the research objects,and they were randomly divided into a control group (n=43) and an observation group (n=43).The control group received conventional comprehensive western medicine treatment,and the observation group received modified Yiqi Yangyin Huayu Decoction on the basis of the treatment of the control group.2 weeks is a course of treatment.After 4 courses of treatment,observe the clinical efficacy,TCM syndrome score,blood glucose-related indicators,C-reactive protein (CRP),homocysteine (Hcy),and renal function-related indicators.Results:The total clinical effective rate of the observation group was 81.40%,which was significantly higher than the 60.47% of the control group (P<0.05).The scores of mental fatigue,weakness of waist and knees,warmth of hands,feet,and thirst in the observation group were lower than those of the control group at 2 weeks,4 weeks,and 8 weeks after treatment (P<0.05).There was no significant difference in fasting blood glucose (FBG),2h postprandial blood glucose and glycosylated hemoglobin (HbA1c) levels between the two groups after treatment (P>0.05).The levels of CRP and Hcy in the observation group were significantly lower than those in the control group after treatment (P<0.05).The blood creatinine (Scr),urea nitrogen (BUN) and urine microalbumin excretion rate (UAER) levels of the observation group after treatment were significantly lower than those of the control group (P<0.05).Conclusion:Modified Yiqi Yangyin Huayu decoction has a good effect in treating early and mid-term DN with Qi and Yin deficiency and blood stasis syndrome.It can effectively improve the TCM syndrome score and renal function of patients,and improve the body’s micro-inflammatory state.
