不同曲度夹板固定治疗伸直型桡骨远端骨折的临床研究

摘　　要：目的:观察不同曲度夹板外固定治疗伸直型桡骨远端骨折的临床效果。方法:选取2019年5月—2020年3月该科收治伸直型桡骨远端骨折患者96例,运用平乐正骨手法复位后,随机分为3组即分为30°夹板固定治疗组、35°夹板固定治疗组、40°夹板固定治疗组。各组患者于治疗后1周、2周(若在两周内复查见骨折移位,及时再次调整复位)、3周、1个月随访拍片检查,予以影像学评定,治疗后2个月、6个月随访予以腕关节功能评价。结果:35°夹板固定治疗组、40°夹板固定治疗组在治疗后1周、2周、3周、1个月的影像学评定疗效、掌倾角的保持、桡骨高度的维持方面明显优于30°夹板固定治疗组 (P<0.05),40°夹板固定治疗组由于夹板曲度较大,患肢肿胀消除、腕关节早期功能锻炼方面不如其他两个治疗组,35°夹板固定治疗组治疗2个月及6个月后随访腕关节功能评价明显优于30°夹板固定治疗组、40°夹板固定治疗组(P<0.05),治疗后6个月随访腕关节功能,各组患者差异无统计学意义(P>0.05)。结论:35°夹板固定治疗组治疗伸直型桡骨远端骨折有利于桡骨高度的维持,骨折复位后稳定性的保持且有利于患肢肿胀较快消除,早期功能锻炼,腕关节功能恢复。Objective:To observe the clinical effect of different curvature splint external fixation in the treatment of straight distal radius fractures.Methods:From May 2019 to March 2020,96 patients with extension type distal radius fractures were selected from this department.After reduction using Pingle bone setting method,they were randomly divided into 3 groups,namely 30° splint treatment group and 35° splint treatment group.° Splint fixation treatment group,40° splint fixation treatment group.Patients in each group were evaluated by imaging at 1 week and 2 weeks after treatment (if fracture displacement is found within two weeks,adjust the reduction in time),3 weeks,and 1 month follow-up filming examinations,and imaging evaluations.2 months after treatment,The wrist joint function was evaluated at a 6-month follow-up.Results:The 35° splint fixation treatment group and the 40° splint fixation treatment group were significantly better than 30 in terms of imaging evaluation,maintenance of palm inclination,and maintenance of radius height at 1 week,2 weeks,3 weeks,and 1 month after treatment.Splint fixation treatment group (P<0.05),the 40° splint fixation treatment group is not as good as the other two treatment groups in terms of splint swelling due to the larger curvature of the splint,and early functional exercise of the wrist joint.The 35° splint treatment group is treated 2 The follow-up wrist function evaluation after 6 months and 6 months was significantly better than the 30° splint fixation treatment group and the 40° splint fixation treatment group (P<0.05).There was no statistical difference in the follow-up wrist function 6 months after treatment.Academic significance (P>0.05).Conclusion:The 35° splint fixation treatment group for the treatment of straight distal radius fractures is beneficial to maintaining the height of the radius,maintaining stability after fracture reduction,and quickly eliminating the swelling of the affected limb,early functional exercise,and recovery of wrist joint function.
