摘  要:目的观察穴位贴敷治疗后支气管哮喘患者血清内源性代谢物的变化,探讨穴位贴敷疗法对支气管哮喘的调节机制。方法收集50例哮喘患者穴位贴敷治疗前、治疗后及50例健康人的血清样本,采用超高效液相色谱/四极杆飞行时间质谱(UPLC-Q-TOF/MS)技术对各组血清进行检测,利用主成分分析(PCA)和正交偏最小二乘判别分析(OPLS-DA)筛选出血清中差异性代谢物。结果健康人、哮喘患者穴位贴敷治疗前、治疗后的血清能够得到明显区分,共鉴定出5个潜在的血清生物标志物(VIP>1,P<0.05)。其中,芥酸酰胺和丙酰左旋肉碱在治疗前呈上升趋势,治疗后下降,13-顺式-阿维A酸治疗前呈下降趋势,治疗后回升。结论穴位贴敷治疗可使哮喘患者的异常代谢有所恢复,可能是通过对机体内5个潜在的血清代谢物及其代谢通路的调节达到治疗的目的。Objective To investigate the changes of endogenous metabolites of patients with bronchial asthma treated by acupoint sticking therapy(AST),to screen metabolic biomarkers related to the treatment of bronchial asthma,and to explore the regulating effect and possible mechanism of AST on bronchial asthma.Methods Serum samples of 50 asthmatic patients before and after AST and 50 healthy controls were collected,and were determined by ultra-high performance liquid chromatography coupled with quadrupole time-of-flight mass spectrometry(UPLC-Q-TOF/MS).Principal component analysis(PCA)and orthogonal partial least squares discriminant analysis(OPLS-DA)were used to find serum differential metabolites.Results The serum samples of healthy controls and asthma patients before and after AST could be clearly distinguished,and 5 potential biomarkers were identified(VIP>1,P<0.05).Among them,high serum level of erucamide and propionyl l-carnitine before AST were decreased after AST,while low serum level of 13-cis-avia-acid before AST were increased after AST.Conclusion AST can restore the disturbed metabolism of asthma patients,and its therapeutic effect may be related to the regulation of 5 serum metabolites and their related metabolic pathways.
