超声半定量评分对乙型肝炎患者肝纤维化分期的判断价值分析

摘　　要：【目的】探讨超声半定量评分在乙型肝炎患者肝纤维化分期中的判断价值。【方法】选择我院收治的乙型肝炎患者78例,患者均采用超声半定量评分检查,并以病理学检查结果作为“金标准”,将超声半定量评分与“金标准”进行比较,分析超声半定量评分在乙型肝炎患者肝纤维化患者中的诊断价值;测定患者肝功能水平,并与超声半定量评分进行相关性分析。【结果】超声半定量评分检查乙型肝炎患者确诊肝纤维化54例,与“金标准”鉴别符合率为85.90%;乙型肝炎肝纤维化S3患者各项超声半定量评分均高于S2期与S1期(P<0.05);乙型肝炎纤维化患者S3期ALT、AST及TBiL水平均高于S1期和S2期(P<0.05);乙型肝炎纤维化患者ALT、AST与超声半定量评分呈正相关(P<0.05)。【结论】超声半定量评分用于乙型肝炎患者肝纤维化中具有较高检出率,不同肝纤维化分期下超声半定量评分存在差异,与肝功能水平存在强相关性。【Objective】To investigate the value of semi-quantitative ultrasound scoring in the staging of liver fibrosis in patients with hepatitis B.【Methods】The 78 patients with hepatitis B in our hospital were selected.All patients were examined by ultrasonic semi quantitative score,and the pathological results were taken as the"gold standard".The ultrasonic semi quantitative score was compared with the"gold standard",and the diagnostic value of ultrasonic semi quantitative score in patients with liver fibrosis in patients with hepatitis B was analyzed;The level of liver function was measured,and its correlation with ultrasonic semi quantitative score was analyzed.【Results】Fifty-four patients with hepatitis B were diagnosed with hepatic fibrosis by semi quantitative ultrasound score.The coincidence rate with the"gold standard"was 85.90%;The semi quantitative scores of S3 patients with hepatitis B liver fibrosis were higher than those in S2 and S1(P<0.05);The levels of ALT,AST and TBIL in S3 phase were higher than those in S1 and S2 phase(P<0.05);ALT and AST were positively correlated with semi quantitative ultrasound score in patients with hepatitis B fibrosis(P<0.05).【Conclusion】Ultrasound semi quantitative score has a high detection rate in liver fibrosis in patients with hepatitis B.There are differences in ultrasound semi quantitative score under different stages of liver fibrosis,which is strongly correlated with the level of liver function.
