摘　　要：【目的】探讨院外延伸护理服务对留置胃管患者的应用效果。【方法】选择于我院留置胃管的96例患者作为研究对象,采用随机数字表法将其分为两组,各48例。对照组采用常规护理,观察组联合院外延伸护理服务,4周护理后对患者效果进行评估,并完成6个月随访,比较两组生活质量、知晓率、满意度、导管相关性感染、非计划性拔管及再入院率。【结果】两组护理4周后生活质量得到明显提高;观察组护理4周后生理因素、心理因素、社会关系、环境影响及总分均高于对照组(P<0.05);观察组护理4周后胃管留置目的、更换方法、注意事项知晓率和护理方法、服务态度、护患沟通满意度高于对照组(P<0.05);观察组护理4周期间导管相关性感染、非计划性拔管发生率及随访6个月再入院率低于对照组(P<0.05)。【结论】院外延伸护理服务有助于提高留置胃管患者生活质量,能获得较高的知晓率和满意度,有助于降低导管相关性感染、非计划性拔管及再入院率。【Objective】To explore the effect of out-of-hospital extended nursing service on the re-admission rate of patients with indwelling gastric tube.【Methods】Ninety-six patients with indwelling gastric tube in our hospital were selected as subjects.They were randomly divided into two groups,with 48 cases in each group.The control group used conventional nursing care,and the observation group combined with out-of-hospital extended nursing service.After 4 weeks of nursing,the patient's effect was evaluated and a 6-month follow-up was completed.The two groups were compared with quality of life,awareness rate,satisfaction,catheter-related infection,and unplanned extubation and readmission rate.【Results】The quality of life of the two groups was significantly improved after 4 weeks of nursing;After 4 weeks of nursing,the physiological factors,psychological factors,social relations,environmental effects and total scores in the observation group were higher than those in the control group(P<0.05);After 4 weeks of nursing,the awareness rate of gastric tube indwelling purpose,replacement method,precautions,nursing methods,service attitude and nurse-patient communication satisfaction in the observation group were higher than those in the control group(P<0.05);The incidence of catheter-related infection,unplanned extubation and readmission rate after 6 months of follow-up in the observation group were lower than those in the control group(P<0.05).【Conclusion】Out-of-hospital extended nursing service is helpful to improve the quality of life of patients with indwelling gastric tube,obtain high awareness rate and satisfaction,and reduce the rate of catheter-related infection,unplanned extubation and readmission.
