百癣夏塔热片联合紫外线照射治疗老年银屑病的疗效及其对血清炎症细胞因子水平的影响

摘　　要：目的探讨百癣夏塔热片联合紫外线照射治疗老年患者银屑病的疗效及其对血清炎症细胞因子水平的影响,为其治疗老年患者银屑病提供依据。方法选取2019年6月—2021年5月于广饶县人民医院皮肤科诊治的老年寻常型银屑病患者90例,采用随机数字表法分为观察组(n=45)和对照组(n=45)。对照组给予紫外线照射治疗,观察组在与对照组相同治疗的基础上,加用百癣夏塔热片治疗。观察并比较2组疗效、中医症状改善情况、血清TNF-α、IL-6、IL-17、IFN-γ水平和外周血T淋巴细胞亚群水平的变化,并比较2组不良反应率。结果观察组治疗总有效率明显高于对照组(93.33%vs 75.56%,P<0.05)。治疗后,观察组皮疹颜色、灼热感、瘙痒和心烦易怒评分均明显低于对照组(P<0.05),血清TNF-α、IL-6、IL-17和IFN-γ水平均显著低于对照组(P<0.05),外周血CD3+、CD4+、CD4+/CD8+水平均明显高于对照组(P<0.05),而CD8+水平明显低于对照组(P<0.05)。2组的不良反应率差异无统计学意义(P>0.05)。结论百癣夏塔热片联合紫外线照射治疗老年银屑病的疗效显著,能有效改善患者的症状,抑制炎症反应,调节机体免疫功能,且安全性较好。Objective To explore the curative effect of Baixuan Xiatare tablet combined with ultraviolet irradiation in the treatment of psoriasis in elderly patients and its effect on the level of serum inflammatory cytokines,so as to provide basis for the treatment of psoriasis in elderly patients.Methods A total of 90 elderly patients with psoriasis vulgaris treated in Department of Dermatology,People’s Hospital of Guangrao County from June 2019 to may 2021 were enrolled and randomly divided into observation group(n=45)and control group(n=45).The control group was treated with ultraviolet radiation,and the observation group was treated with Baixuan Xiatare tablet on the basis of the same treatment as the control group.The curative effect,improvement of TCM symptoms,the levels of serum TNF-α,IL-6,IL-17,LFN-γand peripheral blood T lymphocyte subsets in both groups were observed.The incidence of adverse reactions was compared between the two groups.Results The total effective rate of the observation group was significantly higher than that of the control group(93.33%vs 75.56%,P<0.05).After treatment,the scores of lesions color,burning sensation,itching and restlessness and irritability of the observation group were lower than those of the control group(P<0.05),the levels of serum TNF-α,IL-6,IL-17 and IFN-y were lower than those of the control group(P<0.05),the levels of peripheral blood CD3+,CD4+and CD4+/CD8+T cells were higher than those of the control group(P<0.05),while the level of CD8+T cells was lower than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Baixuan Xiatare tablet combined with ultraviolet irradiation has a significant effect on psoriasis in elderly patients.It can effectively improve symptoms of patients,inhibit inflammatory response,regulate immune function,and has good safety.
