摘　　要：报告1例界限类偏瘤型麻风伴Ⅱ型麻风反应。患者男,53岁。躯干、四肢红斑及结节1年,脓疱2周。皮肤科检查:双侧眉毛外缘稍稀疏;躯干及四肢为主见较多大小不一的红斑和结节,部分皮损中央可见脓疱,结节质中,浸润感明显。皮损组织病理检查:表皮大致正常;真皮乳头显著水肿,真皮全层见密集组织细胞、泡沫样细胞浸润,另有大量淋巴细胞、中性粒细胞浸润及核尘,部分血管坏死。抗酸染色示较多红色短棒状抗酸菌。诊断:界限类偏瘤型麻风伴Ⅱ型麻风反应。A case of borderline lepromatous leprosy complicated type 2 reaction is reported. A 53-year-old male presented with macules and nodules on trunk and limbs for 1 year, and pustules for 2 weeks. Physical examination revealed slightly sparse eyebrows, and multiple infiltrative erythema and nodules of different sizes over trunk and limbs with medium density and obvious infiltration. Pustules could be seen in the center of some lesions. Histopathological examination showed edema of dermal papilla, infiltration of histiocytes and foamy cells in dermis, as well as lymphocytes, neutrophils, and nuclear dusts.Necrosis of vessels could also be seen. Acid fast staining showed red, rod-like bacillus. The diagnosis was borderline lepromatous leprosy complicated type 2 reaction.
