5-氨基酮戊酸光动力疗法治疗肛门尖锐湿疣所需药物浓度的研究

摘　　要：目的观察3种不同浓度5-氨基酮戊酸(ALA)溶液行光动力疗法(PDT)治疗肛门尖锐湿疣(CA)的疗效及不良反应。方法将155例肛门CA分为肛周组和肛内组,然后每组患者随机分3个亚组,分别选择10%、20%和30%ALA-PDT,治疗间隔7~10 d/次,一共3次后判定疗效和不良反应。结果肛周组完成治疗后3组有效率分别为19.05%、58.62%和86.67%,3个月后的复发率分别为50.00%、35.29%和26.92%;肛内组完成治疗后有效率分别为50.00%、84.00%和89.29%,3个月后复发率分别为36.36%、28.57%和20.00%。155例患者未出现感染、瘢痕。结论ALA-PDT清除肛周CA所需药物浓度为30%效果最好,清除肛内CA药物浓度20%组与30%组疗效差异无统计学意义,可选择20%浓度治疗,各组间复发率差异无统计学意义。Objective In order to study the efficacy and recurrence of three different concentrations of topical photodynamic therapy with 5-aminolevulinic acid(ALA)for anal condyloma acuminatum(CA).Methods Totally 155 patients with anal CA were randomly divided into perianal group and anal canal group,and each group was rondomly divided into three subgroups(10%,20%and 30%)according to the concentrations of the 5-ALA.The treatment interval was 7 to 10 days each time.The curative effects were determined after three times.Both groups were followed up for three months.Results In perianal group,the total effective rates of 10%ALA group,20%group and 30%group were 19.05%,58.62%and 86.67%and the total recurrence rates of three months were 50.00%,35.29%and 26.92%respectively.In anal canal group,the effective rates of 10%ALA group,20%group and 30%group after treatment were 50.00%,84.00%and 89.29%,and the total recurrence rates after three months were 36.36%,28.57%and 20.00%respectively.All patients had no infection or scar.Conclusion The optimal concentration of 5-ALA photodynamic to clear perianal CA is 30%.There are no significant differences in clearance rate between the 20%and 30%groups in the treatment of CA in the anus canal,and recommend choosing 20%.There are no significant differences in the recurrence rate among the groups.
